Panicking- DS2 has blood on toilet paper when wiping after poo

Can anyone help put my mind at ease or give me an idea of what this could be, I’ve got a doctors appointment later today but I’m really panicking. DS2 is 4 and over the past week or two has had blood on the toilet paper when he wipes after a poo. He hasn’t really complained about it hurting other than when he has a poo and has mentioned it being itchy a couple of times. Google says it’s likely a slight tear after being constipated and he isn’t and hasn’t been constipated, his poos are very soft. Apologies for the TMI, any suggestions welcome.

If it’s itchy could he have worms? Maybe he has scratched it and caused scabs that come off when he poos. I would try worm medicine

Sounds like a cracked anus due to a particularly large bowel movement.

Will get better over next couple of days as long as no further large bowel movements to exacerbate !

Thanks for your reply, he only really mentioned it being itchy at first and now just says it’s sore when he goes for a poo. I checked his poo and bum for signs of worms and couldn’t see anything and also can’t see any scabs or anything that looks like it’s causing the bleeding but will ask doc later if it’s worth trying worm medicine. Thank you x

That’s what google said too but he normally calls us to check he’s wiped properly (or at all) after he’s had a poo and he hasn’t had any particularly big ones!

Would it help to put sudocreme on overnight and after every poo, might help to heal up any slight tears?

I’d put money it being a little year and then exacerbated and will stop very soon.

That’s what we’ve been doing and it seems to stop for a day or so and then start again. DH said it was quite a lot on the tissue this morning.

Am glad no one thinks it’s anything terribly serious, I’m hoping I’m possibly overthinking it .

Of course go to the doctor but agree it sounds like a split due to a hard poo (called an anal fisure) which the doctor could very easily diagnose by having a quick look if it is that.

Bright red blood is normally caused by piles or tears. Worrying bleeding from the bowel looks more like coffee grounds and more burgundy in colour. Good to get it checked but bright red blood I wouldn't panic about.

So update on the situation, the doctor on Thursday said it’s likely to be a bit of a tear from a big poo that’s being exacerbated and prescribed a laxative to soften everything coming out to make it more comfortable. Took the laxative on Thursday and since then DS keeps saying he feels like he needs to pop but can’t and ends up screaming hysterically in pain (whereas before he just mentioned it was sore and wasn’t really bothered). When he has managed to get a poo out, it’s soft but huge and there seems to be more blood. My plan is to keep going with the laxative over the weekend to give it a chance but if no improvement to take him back on Monday as surely the laxative shouldn’t make it worse??

keep an eye if the blood continues and it becomes more frequent and is going a re times a day then go back

Even when the poo is soft, if it’s big it can still cause tears on the way out. We’ve had this issue with DS1. His poos are massive, and he gets some blood on the paper on occasion.

It's likely it's only an anal fissure. They are very common.

I think this is the problem with my DS, his are very soft but enormous and now because he’s worried it’s going to hurt when he goes for a poo, I think he’s maybe tensing up which isn’t helping.

So another update, the bleeding didn’t stop, there were odd days where it seemed less and then it would come back. DS was also getting very upset saying that it hurt. I went back to the docs with him and they looked and said they still couldn’t see any tears so ordered a stool sample and blood tests. Stool sample has been done and sent off but the hospital can’t get him in for blood tests until 26th April! I’m still giving him the laxative but he’s so upset and distressed when he needs a poo, it’s been awful 😔 other than reassurance and sudocrem any advice on how to calm him down when he needs a poo would be appreciated

I thought the 26th April was ridiculous too 😕 I’m hoping the stool sample shows something so we get an answer and can do something. It’s horrible seeing him in so much pain when I can’t really do anything to help him.

DS stool are soft too which is why I don’t really think it’s a tear/fissure. I hope you get some answers soon, it’s an awful situation.
