High temp - when to worry

DS1 (5) has had a temperature since Sunday night (so 48 hours now). He is prone to high temps when ill but I’m a bit anxious.
Temperature has been up to 40.6 and has been over 40 several times. Calpol is making no difference but ibuprofen is bringing it down. However we can only give that 3 times in 24 hours and it’s only lasting a couple of hours before it climbs again.

He vomited on Sunday night but I think that was a response to his temperature shooting up, rather than part of whatever he’s ill with, as he hasn’t been sick again. He is drinking and weeing. He has eaten really tiny amounts - little mouthfuls of dry cereal here and there.

Covid test negative. Has complained of an occasional headache, dizziness (said the room was tilting) and looks flushed. No rash, no diarrhoea, no nose or throat issues. He does have very dry cracked lips but he’s also prone to these in cold weather and had dry lips last week, so I’m assuming it’s that persisting.

He is very tired and lethargic.

I don’t know. As I write it, it seems like a virus he just has to ride out but I’m just worried.

He has had a swollen tonsil on one side for months now but the GP looked at it a while ago and said it’s a post covid thing from when he had it in January and not to worry.

Would you worry about a temp like this? He doesn’t seem terrifyingly unwell but definitely not himself. NHS seems to say only to worry about temps if they last 5 days+.

I don't think this is the right place to ask, there's far too many differentials at play.

If you call 111 you'll likely get referred to the ooh GP for a callback within the next few hours dependant on other symptoms

I think a child with a temp that doesn't respond to paracetamol or has a short reprieve with Ibruprofen should be seen by a health professional.
