Tiny, squidgy frequent painful poos

My toddler is having this. The poos are of a decent/softish (but not runny) consistency, and absolutely agonising from the look of it.

Most of them are small/smears, but with a decent size poo one of twice a day. In all, this is happening 15+ times a day.

She has other health problems, but nothing that should be causing this (though sure did have recurring c diff until recently). The hospital are aware and if some extra fluids don't work they'll take a look, but are initially stumped for an answer.

So on the offchance anyone else has come across this, I thought I'd ask.

Hi@Somuchgoo have you tried movicol? My two year old was struggling with what looked like very painful poos (often just a tiny smudge in his nappy after a lot of straining) and movicol did the trick.

Do you think she might be trying to stop the poo coming out rather than be struggling to pass it?
My DD has a withholding issue, at her worst she was holding on for 6 days with lots of bouts of screaming when she was trying to stop it coming. She’s on movicol now and it’s under control.

Is it toddler diarrhoea? My son had this for months and months - it isn't serious but it does last a long long time. He had a special diet that iirc was low fibre, low sugar, high fat and excluded some foods eg legumes. He is totally fine now.

She's still on the extra fluids (she has a ng tube so easy to give her extra) that either briefly but then it came back, at now we've added movicol.

The hospital have examined her and she's not constipated, and they seem pretty confused. We are hoping that if she's sore inside that things being softer (though they are pretty soft) might give her body time to heal.

She's been to hell and back this last 6 months (she's had 8 general anaesthetic in that time for example) and I'm paranoid that I'm missing something.

I think there's an element of her trying to keep it in, but she's never been bothered by poop before, and she's had bouts of pretty bad constipation before, followed by a few months of diarrhea, move of which upset her and then everything was finally fine for about 2 weeks before this started up.
