Advice please blood in nappy

Hi , apologies if this is a bit long I was just after a bit of advice/reassurance.
My son is 2 in May , last year in April when he was 11 months old he had a small 20pence piece size of bright red blood in his nappy one morning, I called the gp and had a phone consultation as we couldn't go in due to the new covid restrictions, she said as he seemed well to just assume he had maybe scratched himself and to leave it be. Then a few months later in August it happens again , a bit more blood this time, so called the doctor again who sent of a urine sample, there was no sign of infection so they referred us to the urology department at the children's hospital.
We went to see them in November who checked him over and said he seemed well but they would book an ultrasound.
Finally got the ultrasound appointment in February last week and they said as he hadn't had any bleeding since August 2020 and he had an ultrasound at 4 weeks old (due to other reasons) and everything was okay then they wouldn't do another one but to call back if it happens again.
So I was happy and thought it had all stopped but then a couple of days ago it happened again! Called the GP who said they would write to the hospital but they may not do anything as he is fine in himself.
He really is completely fine in himself and this has only happened three times since last April.
He is my first child so I don't know if it's common? This is only in wet nappies , I have never seen it in a poo nappy.
He is completely healthy otherwise .
Shall I push for him to be seen again? Shall I just wait and see if it happens again? The gp said if it does happen again then take him to childrens A&E so I will do that.
I'm just wondering if anyone else has experienced this?

Just to add there has never been any stretch or cut on his private parts that I can see anyway, and the specialist said foreskin ect all appearred normal.

This sounds so stressful for you - I’ve never had this with either of my twins and it must be concerning. It’s good news that he seems well in himself and it’s infrequent and that his urine test was normal. I would definitely want to get to the bottom of it though.

I am not a medical professional so I can’t say what it might be, but if it’s bright red blood that looks like blood that suggests the problem isn’t in his bladder or bowel as this would then mix with wee or poo rather than appearing as fresh blood.

One of my twins severely bit his tongue twice in his sleep creating a nasty injury to his tongue - the blood mixed with the saliva and was absolutely everywhere, and it didn’t really look like bright red flesh blood as it was diluted.

I still don’t know what it happened and obviously worried about things like seizures but it’s been at least 18 months and hasn’t happened again.

You say they’ve checked his foreskin etc - have they checked his bottom to see if there are any little tears or anything, especially if he is ever constipated?

Other than that I would be asking to see a paediatrician who can order some tests and refer on as needed.

Really hope you get some answers - it’s so difficult when things happen sporadically to find out what’s going on. If it’s just happened again have they done another urine sample just in case?

Thankyou for your reply. I was worried I was being silly , I think I will try and speak to a different GP as they all seem to have a slightly different opinion. They might give the hospital a bit of a nudge! Yes I can't see any little cuts ect. No one else seems to be worried as it is so sporadic, but it can't be normal!

Well something must be bleeding and for it to happen sporadically like this with long gaps between is quite strange so I would definitely want to make sure they’d checked everything out! Does he have constipation sometimes? That’s the only thing I can think of - occasional constipation causing a little tear that bleeds. If it’s soaked into the nappy and spread then it may look like more than it is i suppose.

I hope you can figure it out!

Yes he does have constipation sometimes , it usually sorts itself out with in a day but this is very good point! They checked his bum the first time he went but not last time. Thankyou

I know it’s easy to forget what nappies were like the day before etc, or how long it’s been - maybe keep a note of when he poos, and whether it’s hard so you can see if these incidents link to that, while you’re waiting to see someone? At least then if they ask you you can tell them whether there’s a link or not.
