14 week old- is there something wrong?

My 14 week old has never been a particularly happy baby. She has been in the sling a lot as she doesn't like to lie down on the playmat/ in her rocker as she wants close contact. Up until 4 weeks ago I was exclusively breastfeeding her. 4 weeks ago she suddenly didn't want to latch on to my boob so we offered her a bottle and she had 1 oz. Since then I have been doing a combination of bottle and breastmilk. However, whichever way I try and feed her she has become SO upset and irritated. She will cry however I try to feed her, and when I breastfeed her she will take a few sips and cry. I am having to put the dummy in her mouth to calm her down, then she takes a few sips, then dummy and so on.
She has been so grumpy over the past few weeks and has cried so much. She sleeps for about half an hour at night then will wake up, I then feed her or rock her, put her down and then half an hour she wakes again. Since birth she hasn't slept for more than 2 hours straight. I am exhausted and absolutely fed up, I thought it would start to get easier now and thought the crying would get less but it isn't getting any better.
Is this normal baby behaviour or do you think something could be wrong? Thank you for any help or advice.

Also, the health visitor did come 2 weeks ago to measure her weight and she is following the 50th percentile, so she said she must be feeding enough. I just don't understand why breastfeeding/giving her a bottle makes her so upset :( it is heartbreaking to see and I feel like such a rubbish mum

It sounds a lot like reflux, its worth speaking to the gp about it and maybe talking about medication. As a start a thickener like gaviscon or carobel might be enough to help, but omeprazole reduces the amount of acid in the tummy so could help. Youre definitely not a rubbish mum though! You've done amazing to get her to be gaining weight even with it being hard to get the milk in!

Speak with GP. It sounds like she is not comfortable feeding. Enquire about silent reflux and milk allergies.

I had a telephone consultation with the doctors and he has prescribed gaviscon. Fingers crossed she won't be in pain soon. Thank you for both your eplies.
