Possible UTI

Just wondering if I could get the opinions of some wonderful mumsnetters?

DD aged 3.9 has been a bit quiet the last couple of days. Weeing more than normal, quite often doing one then wanting to go back and try again a few mins later.

Last night was really hot and irritable - temp 38.3, started complaining of tummy ache. Calpol given and pooped her to bed, woke up an hour later screaming her tummy hurt and was sweating.
Had a restless night but slept through and woke up this morning instantly tearful that she had tummy ache and felt “hot and funny”

Ate breakfast as normal tried to poo and really huffed and puffed and strained before producing some small dry poos. Wondered if it was constipation but Then it doesn’t explain fever. She quite often gets constipated when poorly.

By lunchtime she kept grabbing her crotch and whinging. Asked if it was sore and she got all upset and kept saying “my wee keeps trying to come out and go in to my pants and I don’t want it to” instantly made me suspect UTI. On the way home in the car she cried out and told me when we got home she needed some new pants on....Have some dip Stick tests so Did one and it’s showing positive for leukocytes and blood in wee but negative for nitrates.

So do we assume UTI? She has had them in the past but only diagnosed when she was much further gone (ie 40 degree fever and writhing in agony) is this what the early stages are like and how it would present?

Will contact GP as its Friday and don’t want to get worse over the weekend, but concerned I’m wasting their time; and also thought nitrates also needed to show as positive on a dip for a UTI? Or am I mistaken?

She is clearly not happy as is very lethargic and clingy (rather than her usual part ninja part monkey self!🤦‍♀️)

Sorry it’s a bit long and rambly but didn’t get an awful lot of sleep and am feeling a bit frazzled and unsure/doubting myself here!

Its possible to have a uti without nitrites on dip, especially if she is going to the toilet very regularly as the urine doesn't have a lot of time in the bladder. The presence of leukocytes is very suggestive of a uti. You definitely wouldn't be wasting the gps time by getting her checked out.

@IwillrunIwillfly thanks for your reply, I really appreciate it!
I called the GP who said her symptoms fitted the pattern of a UTI and was happy to go with my dip tests results and treat as such. Said he would prescribe 3 days of Tremithroprem.
Collected from pharmacy who said he had actually prescribed 7 days, so not actually sure how long she should take them for.
She is a bit brighter today but still horribly pale and keeps clutching her bits saying wee is still trying to get out in to her pants....will try for the toilet and no wee...think it’s frustrating and upsetting her. She says her tummy isn’t hurting which is good but she looks dreadful. Temp high 37’s.
Re dipped her wee and still showing leukocyte, protein and blood.
Do you think we are on the right tracks? Would you take the full 7 days of ABs or should it be 3 as he originally said? Guess I could call them on Monday to check. Hoping she gets over this easily as she is back at preschool for the first time in 6 months this week and we go on holiday a week today 🤦‍♀️🤦‍♀️🤦‍♀️

Since she's had previous uti's and she's had high temps I would probably be inclined to do the full 7 days, but definitely no harm in seeing how she is on Monday and discussing with the gp. Hope she feels better soon! Im sure you're already doing it but make sure she's drinking plenty!

Constipation can sometimes be the cause of uti's so might be worth keeping an eye on?

Still far from her happy self and temp fluctuates between normal and 38 still. Eating but not as well as normal and playing but still irritable tired and emotional
Pushing fluids as per your suggestion and she doesn’t seem to be going as often but still seems to get the odd sensation that she might wet herself. Only complained of tummy ache once today.
3 days of ABs taken and she is far from herself so totally going to continue with them but thought she’d be much better by now so feeling a bit disheartened. We go on holiday on sat. ABs finish Friday. Have visions of it flaring back up next week and spending half our holiday with a poorly child and on hold to 111 🤦‍♀️🤦‍♀️🤦‍♀️
