Asthma - initial presentation and diagnosis?

I was hoping those of you with experience of childhood asthma might be able to help/share your experience. We're currently querying whether my son (almost 12) has asthma. He started around 5-6 weeks ago with a dry, unproductive cough. No wheeze, no phlegm. After a week he started vomiting because he was coughing so much. After a week of this we went to the doctors who said there were some signs of a chest infection (which I disagreed with but that's by the by) and prescribed a week of antibiotics.

After the week, the cough was still there, so we went back to the doctors. She listened to his chest (still no wheeze) and checked his sats which were fine. No peak flow was carried out. She prescribed Montelukast, 1 a night for a month.

After a couple of days the cough was getting worse, especially after exercise, and because my son had PE coming up I called a pharmacist for advice regarding whether I should keep him off PE for the time being. Pharmacist was confused as to why he had been prescribed the montelukast but no inhalor, as montelukast is an asthma treatment but not usually the first port of call. He recommended that I go back to the Dr and get a little more information about what she was thinking. We went back, explained what the pharmacist had said and told her I was concerned that the cough was worse after exercise. She prescribed a blue and a brown inhalor. Brown for morning and night and blue for as and when required. Appointment made for 3 weeks time to review. She was reluctant to diagnose anything at the moment which is understandable.

So that's where we are now. Has anyone else had similar experiences? And what was the outcome? I work for the NHS and often see and treat patients suffering from asthma attacks so I know what we could be facing, and I want to prepare myself. Thank you.

Yeah, I think asthma can present in different ways. My son didn't wheeze and terrifyingly, didn't respond to salbutamol. He had attacks when he was little and his asthma was so different to mine, I took him to various specialists and they all said asthma. So I have concluded that it's not one thing.

Outcome was he has stayed on a oreventer which does seem to take the edge off the attacks and he hasn't been hospitalized for several years now yay.

Well actually, they didn't say asthma. They wouldn't make a diagnosis actually, and then got huffy with me and asked why WOULDN"T you think it's asthma? So, that was weird, you seem to understand the presence of non diagnosis better than me. But we seemed to have eased into just calling it asthma over the years without ever there being an official diagnosis. Sorry to contradict what I said before. The specialists said there was no signs of any other problems to explain the symptoms.

This is exactY what happened with my son who’s 12, exactly the same symptoms, we went to the drs twice before the 2nd diagnosed asthma. His cough was particularly bad at night ever since we’ve had the inhaler he’s hardly coughed at night.

That's really helpful, thank you both.
We've not really had it explained to us when exactly we should use the salbutamol. He can't have it every time he coughs because he is coughing so much. But the individual coughing 'fits', for want of a better word, are brief so by the time he's got the inhalor out and shook it, the coughing has stopped anyway. The pharmacist who gave us the inhalors said that we were welcome to go back this week for him to check that my son is using the inhalors correctly so hopefully he'll shed some light on that for us.
I understand the lack of diagnosis at the moment because it's so new and I suppose could be something other than asthma. But when we go back to the doctor in 3 weeks, if he's still coughing, I'll be pushing for a diagnosis/further assessments etc then.

My sons came on when we were in Disneyland Paris, the last day he had just developed this cough, I waited 14 days because that’s what they say if you go to the drs, we called up and she said it’s probably just a virus. In total he was coughing for 10 weeks before he was diagnosed.

I’m mystified about the diagnosis of asthma. My son has brown and blue inhalers (age 4) but no diagnosis. And the advice we get about using them is sketchy. We’ve learned through trial and error.

I would suggest getting a "spacer" for him to use with his blue inhaler. It's a big plastic tube (kind of!) and you pump the blue inhaler into it then just breathe from the other end, normally. It's MUCH better at getting the salbutamol into their chest. Not sure if you can buy from chemist, but Gp can prescribe.

The other thing in the meantime - we were told you can use up to 10 puffs at one time & for us, if my DS wasn't getting a "productive" cough after a couple of pumps, he did a couple more. It sometimes did take several puffs to start to clear it. You'll hear him kind of coughing up stuff.

Also - if he is doing any exercise, we were advised that DS should have a couple of puffs before he started too. And always keep close by. Exercise-induced asthma can occur too.

Hope any of that helps. I'm not medically trained, but both DS (now 21, diagnosed at 6) and myself are asthmatic.
