Poo witholding

My son is 11 and is autistic.

When potty training (still isn't reliable now) we had issues with him pooping so I let him poo in a pull up and get it off as soon as he pooped in it or he witheld his poo and got constipated

But a few months ago he started wanting his pull up to wee in and to wear it even if he didn't need a poo

They are expensive so I couldn't Afford them just so he could wear them when he didn't need a poo

Monday night I could tell he was withholding as he was trying hard to hold on to it I asked him if he wanted to go on the toilet (he had 2 poos when I stopped pull ups but think he was desperate and distracted) but he shouted no and got wound up

Yesterday and today hes been constipated and straining and holding his belly, he's been upset and irritated aswell

I gave him lactulose(prescribed by gp) yesterday afternoon. But he spat it out so he didn't have any

Help!

I've read that getting a routine if sitting on the toilet if you can is good.
maybe after meals

will he sit on the toilet if bribed (e.g. with ipad)? If you can get into a routine of sitting on the toilet at the same time each day (e.g. 10 mins after breakfast), that might help.

how is his language? talking about what needs to happen and having a visual storyboard could help him understand.

can you get movicol as a stronger laxative? but it tastes worse than the lactulose so he might not take it.

His language is OK.

Movicol made him sick before so was prescribed lactulose

What had you tried giving the movicol in? You may have already tried the following...

Any non-boiling liquid. If he's sensitive to food texture it can help to give it in a liquid that naturally has some texture to it (because the mouthfeel can be a bit odd in a liquid that is normally totally smooth). You can taste it more at room temperature, and less so when fridge cold, or in a hot drink (has some success with hot chocolate). In lower doses, have managed to get some in using jelly, instant whip, and even gravy.
