Please help me finally clear this nappy rash

My ds2 seems to constantly battling nappy rash, just when it seems to have healed it flares up again we are super vigilant with nappy changes as he’s quite routine with his poos he’s literally in a soiled nappy for seconds but it’s almost like he’s allergic to it. A few weeks ago he had hfm which really flared up around his already sore little bum, took him to see the gp and other then confirming what we knew already nothing was recommended. Iv tried suda crem and bepanthen and now onto metanium, which seemed to do the trick until today and now I’m back to square one I feel so sorry for him. Is there anything I’m not doing that’s worth a go? Tia

Dd has faecal impaction with leakage at 18 months and this caused the worst nappy rash, her bottom was bleeding it was horrendous. The Dr at Paed A and E gave us a genius prescription for it which was orabase and yellow paraffin mixed up. 2 parts orabase to 1 part yellow paraffin. He explained the orabase is for mouth ulcers and is designed to stick to wet skin so literally clings to skin even in a wet nappy. It cleared up so quickly once we started using it.

Are you breastfeeding at all?
I was taking a magnesium supplement for vasospasm & couldn't work out why my poor DS had loose stools & hideous nappy rash.

Stopped the supplement, bum healed.

My DD used to have terrible nappy rash. Only caneston would clear it up. Plus we had to use cotton wool and water, no wipes.

Another vote for drapolene here. Dd1 had terrible nappy rash, skin would break down and bleed on wiping Sad.

Drapolene worked within 24hr. Other tips are if wipes or even cotton wool and water are stinging, to use olives oil and water instead. Obviously will need a bath every day, but oil wipes will also clean off poop without stinging.

Other tip is to consider allergy as pp said, for dd1 it was lecture intolerance causing acid poop. Good luck Flowers

After his morning poo il give bare bum a go, I have carpet everywhere and it’s cream so I’m not looking forward to that but I can clean the carpet I can’t clear this horrible rash! I’m not breastfeeding anymore no, it seems to be worse when he has loose poos it’s like acid on his skin.

Bare bum as much as possible.

Clean with a flannel and warm water, or just warm shower

Have you tried using reusable nappies?

I agree with @itsboiledeggsagain its fungal. My daughter suffered terribly but the almost immediate difference an anti fungal made was amazing.

I'd try canesten in case it's fungal (often was in our case). Mix it with the Metanium and possibly try some steroid cream too (0.5% hydrocortisone - check with pharmacist first). Use water and cotton wool, pat dry before clean nappy. Maybe try a different nappy brand (something like Naty which doesn't have added moisturisers etc.). If that doesn't clear it, ask GP to take a swab.

I realise you've had a lot of advice and don't wish to complicate it further, but if I were you I'd be asking Gp about needing an anti fungal cream, you can also get canesten hc which is anti fungal combined with hydrocortisone, used sparingly it cleared up my children's nappy rash fast, once you have the redness/ infection (potentially fungal) under control, you need to maintain a barrier to keep urine away from the skin, so wash (cotton wool and warm water best) then dry (we used muslins) and then apply bepanthen or a barrier cream after every nappy change, and do this frequently. Gentle bath every night, use gentle soaps, eg weleda with calendula.
If dr does prescribe thrush medication etc, then ideally apply and then pop bepanthen on top, if you have time to leave thrush cream to soak in for 10/20 minutes that's preferable, but you need to maintain barrier, so apply bepanthen/ barrier cream even if straight away. It's worth noting that bepanthen because it's not medicated can be used ongoing, metanium is medicated so should only be used when infection is in place and if you are using the yellow metanium it's medicated and not a barrier, so does a different job.

If the fungal cream works don't gi e it up too quickly. I know what people are saying about using it for too long but it is a false economy if you have to keep starting again as it came back. One decent run at it is better than prolonged spurts.

Caneston cream should hopefully work. Persistent nappy rash tends to be fungal.

Change the brand of nappies too! My dad had horrific nappy rash that turned thrush, canesten, metanium and changing from the green brand Tesco nappies to the purple brand really helped (I think they were bigger fitting so less clammy in nappy area?)

Medihoney is great. The wound gel is the best for really bad rash and the barrier cream for maintenance

Could it be the nappies you are using. I had a horrible cycle of nappy rash with dd we’d get it cleared up then it would return again. Someone suggested changing nappies so we tried supermarket own ones and it cleared up. If we even borrowed a pampers nappy off of anyone it came straight back. I often wondered if it was the gel crystals and less cotton wool that was doing it.
