My 4 year old will not poo in the toilet - please help!

Hi

I have posted a while ago about my 4 year old DS struggling with constipation and received some very helpful advice on here. However, we seem to be struggling again. To give some background, my DS has always been a very picky and difficult eater, and he will only eat very limited meals. For example, he will eat spaghetti bolognese, a jacket potato with cheese, plain boiled rice with gravy, Turkey meatballs in tomato sauce, a beef burger, chips. And those are the only meals he will eat each week, so usually I will make spaghetti bol a couple of times a week. I know those meals aren't bad but it's more breakfast and lunch times which are most difficult. He will not eat cereal or porridge of any kind. His breakfast each day is usually grapefruit and a yoghurt. We all like fresh red or pink grapefruit in our house so luckily he likes that, but I think it ruled be vetter if he'd eat some kind of cereal. Lunch times are hard as he will not eat a sandwich of any kind. He likes plain pasta shapes cooked, a dish of plain rice with gravy, sometimes a bit of cheese and tomato pizza. He loves yoghurts and fruit pots, he will eat apples and strawberries.
Sorry for all that info, but just to give an idea. I don't think his eating is brilliant and I think this may be the reason he suffers so often with constipation. Anyway, a few months ago he was badly badly constipated a do think it was 9 days he went without having a poo. He had awful stomachache and the doctor prescribed Movicol, which has been and still is wonderful for him. He had sachets for a couple of days when he was badly constipated and compacted and it did the trick. The Dr said to wean him slowly off the Movicol, but I haven't quite got there. I have reduced it down and down and he seems ok on half a sachet morning and night or just one full sachet before bed, but any less than that or stopping it completely and he just cannot poo.
I'm not especially concerned about him being on Movicol I it helps him, but the problem is that when he has been a couple of times badly compacted with hard poo that's been hard to pass, he got frightened to let his poo come out because of how much it hurt or how much he thought it would hurt that he stopped going on the toilet. Instead he would stand with his legs crossed trying not to let it out. And obviously eventually it would come out in his pants. I have struggled and struggled trying to get him to sit on the toilet and I try 20 mins after each meal to sit with him on the toilet, which he will do but he pushes and pushes and nothing comes out. Then a little later I'll find he's gone in his pants.
My DS starts school in under two weeks and I made it my aim to try and get this sorted out over the holidays but I am just not winning with DS and he's getting upset over it and I am too. Every day he poo's in his pants, and will not do it in the toilet. I have tried everything, reward charts, bribery, taking to him, treats and the promise of a brand new scooter at the end of this week if he manages to do one poo each day on the toilet. But no.. Been out with him and his younger sibling this afternoon to a park and playground and I could see him stood straining with that look on his face where I know he needs to poo but isn't letting it out. I feel ive tried everything. I am worried that at school he will poo his pants, and I don't know what will happen then. Will school send for me to change him? I obviously don't mind doing that but I wish for his sake that he could go more easily and that he'd sit on the toilet. I don't want him to be the boy at school who smells of poo when he's had an accident.
People say the eating more will come but making his lunch box when he starts school will be difficult.
I am planing a trip to the doctor's with DS this week but I'm not sure what they will suggest. I am wondering if he needs a referral to check his bowels are ok. Does anyone have any useful advice or has anyone got experience of this?
Apologies that this is such a long post and I would be grateful for any advice or shared experience.

Hiya, just read through your thread. Well done DS for going on the toilet!! Just wanted to reassure you further that he'll get there. My DS was exactly the same as yours- not fully toilet trained when he started school as he was a poo witholder. We used movicol (didn't get on with lactulose either) and the doctor was very relaxed about him being on it, said to keep it up as long as was needed- its a long process and we eventually did a very slow withdrawal once he was confident pooing on the toilet.

I really wanted to reassure you not to worry about school, they will have come across this many, many times. They will also have had to clean up children who have accidents- it happens to lots of them when they are distracted. I would have a chat with your DS's teacher when it's calmed down a little and let her know he's on movicol. She'll then be able to watch out for him a little and know to encourage him to go to the toilet when necessary.

Finally, my DS went from pooing in his pants fairly regularly to pooing on the toilet every time he sat on it, within the space of a couple of months. He just eventually got it and realised it didn't hurt if he didn't hold on to it. He's now off the movicol completely and has no worries. We did lots of discussion about what happens to the poo when you flush it away as that was another thing that worries him- we said it was going to a poo party with all the other poos, so then he was keen to flush it away saying 'bye bye poo, have fun at the poo party!' Daft little idea, but it definitely helped!

Good luck, try not to get too stressed about to- they definitely pick up on that.

Ds is 3.5 so a bit younger than yours. We give him 1 sachet of movicol a day which seems about right for us. It works by draeing water into the bowel so no problem being on it as long as necessary.

Sounds like your ds has made a real breakthrough which is fantastic news. Hope it continues and all goes well at school. You might be right about the lazy bowel if this has been going on for a while. I think we had that. He used to hold on for several days then there would be an enormous poo overnight. I could never tell if the days of lots of little drips were actually overflow around the constipation. Our experience was that once on the movicol he couldn't withold for long and we started getting normal poos within the first month. Initially we rewarded every poo in the proper place with a chocolate button. Once that seemed sorted we switched to rewarding the clean pants- we were getting skid marks all the time so wanted to stop that. We're nearly there now but do regress when the routine is different or he's distracted. Hope your ds gets there soon!!!

GirlFromTheNorthCountry - Hi and thank you so much for reading and sharing your experience. It really is so helpful to me. DS has been on the toilet 3 times today and had 3 successful poo's. I truly can't believe it after he's only just got back on the potty. I will definitely have a word with jus teacher and explain he suffers with constipation and that he's on Movicol etc. Like your DS, I think mine has finally realised that it's not going to hurt when he goes (on the Movicol anyway) and that's why he's going on the toilet now. The Movicol is brilliant, or it certainly is for my DS. I was going to ask you about how to wean him off it but I know it needs to be a really slow process and I'm on now hurry to do that as I think we'd be back to square one with bad constipation, withholding and going in his pants. I can't tell you what a relief it is. I never thought I'd see the day. Ive been cleaning his pabts out every day in the bath, the one's that are too bad I don't bother cleaning but I'm fed up of washing dirty pants or throwing them away. It's a huge breakthrough. I know it's true what you say about them being used to accidents at school, I just would love it if he manages to go to the toilet. It would be such progress for him. I only saw the doctor last week who was writing off to the hospital, so I'll still get my referral. I'll still go even if he's still going on the toilet as I could do with the advice re the Movicol. He starts school tomorrow, he's just doing 3 hours this week so hopefully he will go for a poo in the morning before school and then not need one until he's home. That would give me a bit longer to get him more comfortable with using the toilet, as it has only been one day.

Cudby- it's great to hear that you're almost there with your DS. One Movicol a day is good too. My DS is on two a day at the moment but hopefully he'll drop down to just one at some point, although I don't dare reduce it at all at the moment. It does seem to make him go for nice normal soft poo's that are easy to pass. If your DS is a bit younger obviously he won't be starting school yet so you'll have that big longer to get things just right by the time school comes round. With my DS going on the toilet today 3 times successfully, I would really have liked the rest of the week at home with him to get him really used to the toilet again, but I'll have to just cross my fingers tomorrow with if being his first day at school and hope there's no accidents... Eek! I will post tomorrow and update on how his first day goes. I have rewarded DS as well, he got a Playdoh set today for starting to use the toilet.

mrsm22 that's fantastic news you hand your DS have made so much progress so quickly.

Please don't be in a rush to reduce the Movicol too soon! As you've spotted as soon as DS gets constipated again he regresses back to being sore, avoiding pooing and going in his pants. He needs a long period of happy poos to break the cycle and to really learn what he's doing and given he's also starting school, he also has to get used to that to.

Plus every time you reduce the bowel has to do more work so reducing can take months or years - it's a beginner's mistake to think 'he's going, problem solved, stop the laxatives' and be straight back to square one again before you know it.

Sorry Ive not been back to this thread-term starting has been manic! That's such brilliant news-well done! Smile ds's first days have been fab, but he's been a nightmare at drop off... Lots to work on there-he is super clingy...

Teacher54321 - fantastic news that your DS has enjoyed starting school, but sorry he's been a bit sad at drop off. It's such a big thing for them even if they have done nursery. Are you able to go see your DS at some point in the day with working at the school?
Today is day two of using the toilet and my DS is still using the toilet for poo'. No accidents as yet. This morning was his first day at school, he had two poo's on the toilet after breakfast before we went to school and he was only there until 12.30 but no accidents and he said when I picked him up that he didn't need to poo while he was at school. We there until 12.30 the rest of this week, so he might not need to go as he comes home straight after his lunch, but he might need to go and if he does need to I'll just have to wait and see what happens. Hopefully he will go to the toilet at school now that he seems to have cracked it at home! Long may it continue. He was very quiet when I left him this morning but no tears. I must admit, my stomach was in knots all morning while he was at school but when I picked him up he'd eaten all his lunch and he'd enjoyed it. He's very tired though.

Wolpertinger - Don't worry, you're absolutely right about the Movicol. I will not be reducing it. I know the bowel takes time to adapt, and he's managing to go easily each day at the moment. The doctor was writing off for a referral so when we go I'll ask for some advice on the Movicol re dosage and slowly weaning off etc.

Hi everyone, I'm not sure if anyone will pop back on here or not but I just wanted to give an update after my DS's first week in Reception at school. He started on Weds so has had 3 mornings at school, he's really enjoyed it, eaten all his packed lunch which is a big suprise but he is still going to the toilet for poo's since Tuesday when he went on the toilet for the first time. I'm so proud of him and the clean pants. It's been a hugely successful week. He hasn't actually needed a poo while he's been at school this week as he's only been in for a few hours each day, which is how they do it at DS's school for the first two weeks. I can only say I hope if he needs to go to the toilet at school that he will go but at the moment he's been going in the morning before school and after tea in the early evening. Really can't believe how much progress he's made. It's nice to be able to give a positive update.

Hi everyone! Thanks ever so much. I am so proud of my DS. I am truly amazed at him and the progress he's made. I would never have believed it. It's week two at school and so far no accidents, but I know he also hasn't needed a poo yet while at school. We have spoken about it and he knows what to do if he needs to go, just to go to the toilet like he does at home and he says he will. This week is just afternoons and his first week was mornings, but next week is full days so I can't see him going all day at school without needing to go for a poo. I just hope he goes, I think I'll feel happier once I know he's beeb to the toilet at school. I know he'd tell me too. Anyway, regardless it is fantastic news and he goes to the toilet every time. He's even stopped using the foot stool he made, even though I've ordered one from ikea which is meant to arrive tomorrow.
He is still on Movicol, the same dose which is two a day and I'm not stopping or reducing as I know the constipation is still a problem. But he's going well.
How is everyone else?
Teacher54321 - I can imagine how full on it is and has been at school for you. How is your DS now? Has he settled any better?
