toilet anxiety?

My dd is nearly 2, and for just over a year she has had problems with pain whilst going to the toilet. Up till she was 9 months she never had a problem, then seemingly overnight she has developed quite severe pains when doing a poo. Sorry to go Into detail, but she isn't constipated, the poo is like peanut butter when she finally goes and is very very soft. I have been to the docs countless times and have been given lactilose on multiple different doses, then we were put on movicol, which increased to three sachets a day. She was finally referred to a specialist who suggested she had a blockage, we followed his instructions, there was no blockage. She had a test that came back completely clear from all allergies and misfunctions. He has now decided that she may be Intolerant to milk, so has taken her off of dairy and taken her off of all the movicol. It started off pretty bad, she didn't go for a week, but then her poo changed colour completely to what I would consider normal, but still runny. She had a few really good days where she pooed normally and it was amazing, we thought problem was solved, but we are now back to having a lot of pain when trying to go. I think she has anxiety as she thinks it's going to really hurt so holds back from going as much as she can. It's almost like she doesn't know how to push it out? I don't know how I can help her learn how to go?

How does she position herself to poo. Squatting deeply is the most natural and beneficial for emptying the bowels. So not on a toilet.

Her body should do the pushing. The change in colour to normal sounds like diary might be the issue as per the medical advice.

How much water and vegetables is she having?

She is not a fussy eater at all, eats loads of fruit and veg, water intake is abit more difficult, she doesn't drink a lot like me, she only drinks coconut water, when it comes to pooing positions, she is usually in so much pain when going the goes rigid trying to stop it happening, I have to make her bend her legs, it's really awful Sad
