20 month old with pain around vaginal / labia

I'm looking for suggestions of what might be going on with 20 month old DD.

Recently she has been complaining on and off of pain around her vagina / labia.

There is no discharge, although the internal lips area do look a bit red.

Her night time nappy is a bit smelly in the morning, but it is a strong wee smell which I've put down to having one nappy on for 12+ hours and, as she recently stopped feeding in the night (December) more concentrated wee overnight (the nappies are much less full in the mornings now she's stopped feeding).

It seems to be an acute pain when touched, or if wiped with something astringent (I've been cleaning her with witch hazel in case it is thrush).

We saw the Dr about something else earlier this week (chesty cold) and she thought it might be thrush. I'd rather not use cannesten on her unless I'm sure that's what it is (the Dr did do a swab but results won't be back until monday).

I reckon it could be one of the following, but none of them make total sense. Any ideas:

Thrush - the area is red, but no discharge, not always smelly, seems to be acutely painful on and off (when touched) but not all the time, and not itchy.

UTI: But nappies / urine are not consistently smelly. (i will drop a sample in to the Drs to rule this out). She has had a cold this week, but only a temperature one day, is otherwise on good form.

Broken hymen: Is this possible? Last week after her bath I noticed what seemed to be a little piece of wipe or something by her vagina, but when I tried to gently wipe it away it didn't come, and as it was uncomfortable for her I left it.
She is super active, running, jumping, climbing sometimes falling, she really enjoys her rocking horse and see saws etc and sitting on those wobbly animals meant for much bigger children. So I wonder if all of that has broken it a bit?
This would explain why it is very painful when touched, or anything at all astringent comes near, and presumably will heal on its own?

I'd really like some advice, obviously i don't want to ignore discomfort in that area, at the same time I'd rather not use anti-fungal creams or antibiotics on her unless I'm sure that's what is needed.

Hi @DougalTheCheshireCat I know it's a long time ago now but did you ever get to the bottom of this as my DD has pretty much identical symptoms!
