NHS child health programme - school

DD has had a form sent home from school asking if we want her to be part of the NHS Child Health Programme. It's a standard questionnaire asking if we have any concerns about development weight etc. (we don't have any concerns).

Does anyone know what they actually check kids for over their school life & any experience of it?

At the moment they check their height, weight (to work out BMI) in reception and in year 6. They also check their hearing and vision in reception. These are the checks that generate the letters where it tells you if your child is a healthy weight, underweight, overweight or very overweight so some parents prefer to opt out. (Ex school nurse to who used to deliver the programme).
