Help! Silent reflux vs cmpi/lactose intolerance both? Calling any experts

My 6 week old dd is fussing when I'm feeding (ebf) she's always cried and is my first so I had no comparator..

She cries, arches her back, is sometimes sick and is a constant feeder. Hours and hours on end...with literally no break.

She also has many many dirty explosive nappies a day, it's not unusual to have 3-5 changes during a mammoth feed...

She seems to need comfort when pooing (hence the nappies during a feed). She's put on lots of weight so I've found GPs to be very uninterested which has p'd me off. I suspected silent reflux and hv has agreed with me, but also suggested a lactose intolerance.

We have some family history of dairy intolerances, to cows milk (me) lactose and eggs. (Db)

Hv told me that as I am bfeeding for any of these I would need to cut out raw dairy, this doesn't make sense to me so interested in hearing from someone with some knowledge in this area?

Also we've started dd on losec mups, there's been no improvement. so far (it's day 3) please can someone tell me it gets better.....

it's really unlikely to be lactose intolerance, that's very rare and she would be pretty ill/failing to thrive if that was the case.. breastmilk is full of lactose!

more likely to be an intolerance/allergy to cows milk protein, which should improve if you cut ALL dairy out of your diet completely.

were you prescribed anything for reflux?

This sounds like my DS. He is allergic to dairy. Cutting out dairy from my diet when breast feeding him made a dramatic improvement within a couple of days, although I think it takes a couple of weeks for all traces to disappear.

I unfortunately replaced dairy milk with soya milk, which he was also allergic to and also was eating lots of eggs, again allergic to.

I would use something like Oatly or KOko as a replacement rather than soya.

Been through this I would advise cutting every trace of dairy and soya from your diet for at least two months and gauge the improvement. My dd was putting on lots of weight too and also CONSTANTLY fed. Turns out she was v allergic to dairy and soy. Good news is at 2.3 now she is able to tolerate tiny amounts in things without a reaction so we don't have to be as careful nowadays. Go onto allergy board for lots of advice re cmp free diets.

Agree extremely unlikely to be lactose intolerance.

Has she ever been checked properly for tongue tie? Much of what you mention can be symptoms of ties.

Health visitor has checked but I'm wondering if she needs a second opinion.... She does have lots of those symptoms and some I haven't even listed below.

Losec MUPS takes a good week or two (at least) to get working. I'd be cautious about changing 2 things at once (eg starting Losec & excluding from diet) as it can't be identified which 1 has had an effect, so the other could be unecessary. Perhaps give it another week or two before changing diet?

Also agree cmpi more likely than lactose intolerance.

I was going to say tongue tie too.

Otherwise could well be a cows milk protein allergy. Kellymom has some good info on BFing & food allergies. 40% of babies that have cmpa ate also allergic to soy. DS has more trouble with soy than dairy. Koro coconut milk or oatly are both better than soya milk anyway.

Primary lacotose intolerance is extremely rare in babies. You may well have some degree of secondary lactose intolerance if something else damages the gut (can happen after a GI infection or if there is an undiagnosed food allergy or intolerance causing inflammation) but in that case you treat the underlying cause and it goes away.
I agree with the others that cows milk protein intolerance is by far more likely. You need to be really strict about cutting dairy out. You may well need a calcium supplement yourself as requirement for BF is 1250mg a day. Read labels like a hawk, and if you need suggestions for replacement products then post again. Also agree re avoiding soya replacements. It takes a few days to clear your system and BM, then a few days to clear baby's system, then days-weeks to allow the inflammation to heal, so you need to be thinking in terms of weeks to see improvement. We were definitely improving within a fortnight, but it took 6-8 weeks to see the full extent of it.
I'm another one with a TT and MSPI (milk and soya protein intolerant) baby - there does seem to be a link between the two.

More likely to a cmp allergy, my DS has one too and he has reflux and eczema.

Putting on weight is irrelevant, my DS did. Some GPs seems to think this is a measure of assessing reflux, it isn't. You need to cut out all dairy from your diet and soya as they can be allergic to that as well.

The omeprazole may take a whole to start working but when it does its brilliant. Are you winding her regularly? Tilt the mattress in her cot and keep upright for 20 mins after a feed. Go back to your GP and get a referral to paediatrician.

I'm based in Cheshire - going to get lo rechecked for tt.

As she was in a lot of discomfort I cut out dairy and soya. I did plan to do one at a time but couldn't wait...

She has gone a lot better-5 days after Losec Mups and 2 after cutting out dairy. But still having some more minor symptoms so will see what happens in a week when dairy reduction kicks in
