Question about young child with (possible) second UTI in short space of time - sorry, long!!

Hello

I'm posting in here as well as 'general health' in the hope that someone can advise me.

DD2 is 35 months old. She started with urination pain in early December. I have urine dipstick tests at home for my own use (another story!), so when she complained of painful weeing, I tested her urine and it tested positive for leukocytes, non hemolyzed blood and a trace of protein. We took her to the out of hours GP as it was a weekend, and she was prescribed a course of ABs. After a single dose, she was much better. She completed the course. In the meantime though, the original urine sample had been sent of to the lab for culture and sensitivity test and that came back negative for bacteria. I tested her urine again to check if whatever she'd had had cleared, but it was still showing positive for the same things.

So, back to the (regular) GP who sent of a second sample to the lab. Thsi time it came back showing mixed bacterial growth amnd we were told this meant that the sample was contaminated. As DD's symptoms were gone, the GP said she wouldn't treat her and leave it to see what happened.

Well, a few days later she was crying when weeing again so another trip to the GP. This time she dipped her urine but it showed very little wrong with it. On this basis, the GP said she thought that perhaps the painful urination was caused by irritation to the vulva - DD does get sore 'down there' sometimes. We left it at that and she was pretty much fine for the next few weeks.

Anyway, last Monday we were away from home for a few days. DD2 got up and went for her morning wee and immediately cried and refused to continue. I checked her vulva and it was very red/sore looking. I put some cream on as advised and by the following day she was urinating as normal, without pain. She continued to be ok for two days, and then last Thursday was complaining of pain again first thing.

I decided to return to the GP to see if they could recommend any better cream - I wasn't really thinking of a UTI at this point. I took a urine sample along anyway and when dipped this showed leukocytes. The GP asked if she'd had a temp and I told him no, just the burning when she wees, on and off for the previous few days. He said it didn't sound like a UTI and gave me more cream for her vulva (which wasn't sore at this point). He did also send the urine sample to the lab to be on the safe side. Fast forward to today and I get a call from GP office to go and collect a prescription for ABs for DD2 - the lab culture showed bacteria and therefore positive for infection.

I spoke to the GP on the phone and he said he is going to look at the guidlines for children with recurrent UTIs and get back to me if it needs to be taken further.

This has left me really confused. Firstly I don't know if she did have a UTI the first time back in December. If not, what did she have? How does that tie in with the sore red vulva which obviously causes pain when she wees? Basically, WTF is going on with her?!

She is perfectly well apart from the painful wees. She isn't weeing more frequently, is dry at night etc...

I'm still not convinced that this current bout is actually a UTI. Is it possible for bacterial growth to come from elsewhere?

Sorry this is so long and drawn out, but it's doing my head in! Has anyone else had this with a child?

I have to say, dipping yourself is confusing things. I would stop doing that. My DD was prone to UTIs and in the end they scanned her for kidney problems...there's something where a tube in there can be too short...leading to troubles. My niece has it....DD was clear and grew out of UTIs.

I found that letting them go nappiless or pantless helps enormously...is she still in nappies? If so, ditch them and let her bits air out a lot.

Watch her when she wipes..or maybe you do that...so ensure she's very well wiped after wees.

Agree that home-testing is hit and miss. DD has a history of UTIs but I've given up on the test strips as they always seem to show something whereas when the culture comes back from the lab there's nothing there, or vice versa.

We have recently been referred for a kidney scan which uncovered a mild abnormality but the urologist felt it could be managed through persuading DD to drink much more and take more time to go to the loo, and go more often, none of which she was doing. DD is 6 though so easier to do all this than with a 3yo.

Thank you both and sorry that you've had to deal with UTIs.

DD has been out of nappies for almost a year, apart from at night when she does the odd unpredictable wee!

I agree that no good comes of dipping at home. It's certainly added to my overall anxiety and confusion.

Anyway I will try getting dd to drink more and make sure she's wiped dry and correctly after wees.

The moment DD used to show one sign of being sore, I upped her water hugely. I used to say "have a drink" every ten mins, I found it really helped and seemed to stop infections in their tracks.

My dd (4) is due to have a kidney scan too because of yet another uti over Christmas. I wipe for her whenever I can, as I fear that this may be at least part of the problem.

She is also very sensitive to soap and shampoo "down below", so I shampoo her hair over the bath (with her standing outside it) and I use E45 Junior for her bath, as this doesn't seem to irritate her like soap does.

I hope you get it sorted for your dd.

gottasmile, your poor DD. I haven't heard back from the GP yet. He said he would let me know if mine needs further investigations. How many infections has yours had?

Since our latest epsiode, I'm trying to make sure DD drinks more, I'm wiping for her, I'm checking her vulva area each evening to see if it looks sore and am looking into giving her a pre and probiotic to protect against the effects of the ABs. Not sure what else to do right now.

Oh yes agree with the no products in bath at all.....I didn't even use soap on DD...she had a soak in plain water then I'd use E45 and a flannel to wash her.

My dd has had 3 infections, which were enough for the gp to wonder if there was a problem with her kidneys. We've had to wait til the end of march for the ultrasound.

i'm trying to be positive and don't really think it's her kidneys, but it will be nice to have peace of mind.

I guess you just keep doing what you've written and then if nothing improves see if they can investigate further. I hope you've seen your last infection, your poor dd.
