DD has had urine infection around 6 weeks, anti b's not working

DD is 6 and for the past 5 or 6 weeks has had tummy aches and pain when urinating. We have had 4 different types of anti-b's (amoxicillin, trimethoprim, co-amoxil and Keflex in order) the tummy pain persists even during the course of anti-b's.

first urine sample was lost. Second one given while on an antibiotic co-amoxil (as pain persisted) came back as clear. Still had pain after course finished by gp said maybe was just the bladder settling down after infection. Then last weekend had to go to emergency gp as she could hardly urinate it hurt so much (a week after co-amoxil stopped) and was given keflex. a dip test was done by emergency dr that showed high nitrites and white blood cells.

I am taking her to gp again at lunch, She is now complaining of dizziness, blurred vision and headache even though still on keflex.

I am very concerned that this is persisting and worried there is something more than a UTi going on. Anyone with experience of this sort of thing got any re-assuring words?

fedupofslugs sorry to hear about your dd. My dd is 13 and is on the third week of a UTI. We had the 4th GP visit today and a 5th appointment next week. She has 7 days of antibiotics again. Each time a sample is taken there seems to be a problem and they ask for another sample.

I have been doing some reading and asked my dd to do the following:

drink water regularly
eat more fibre so no danger of constipation
go to the toilet every 2 hours

FedUpOfSlugs · 20/07/2012 21:05

silver73, sorry that your dd is going through a similar thing. We have been trying to get her to drink more (she certainly drinks more than before UTI's now but not sure if its enough and she is getting very fed up with me trying to force drink down her). She has been having cranberry juice and lemon barley water too.

Hadn't heard the 2hr rule with toilet, been getting her to go more frequently, but will aim for that as a minimum.

GP today was wishy washy, "I'm sure it will get better" and urine dip test clear, so we will just wait and see how next week goes once the antibiotics have stopped again.

If I were you I'd ask to take it further and get her referred for a scan . Your doctor doesn't sound to good my friends child has only had to urine infections and was sent for a scan as only five years old. My eldest dd gets them frequently and went for scan and kidneys were fine . Iv been told a lot of the fruit squash we have now has got something in it that irritates the bladder . Hope your dd is better soon Smile

My dd (18) has issues with repeated UTIs. She had a period when she had them a lot when she was smaller and had a referral/tests but everything seems structurally normal she just seems prone. I would agree with getting your dd referred. Also has her sample been lab-tested and not just dipped, to check which bug it is and therefore the best anti-b?

When dd has a UTI and has anti-bs I always get a retest done afterwards to check it's clear - sometimes the symptoms can settle even though it's not properly gone and it will just flare up again.

We've also had some success with cranberry capsules available from the health food shop/pharmacy. She took them for a few months last summer and they seemed to help. I've just bought some today as she has a borderline UTI at the moment and we're trying to shift it without antibiotics as she has had rather a lot of them and TBH they don't always seem to do ant good as you've found.

Thanks for all the advice. We have a variety of problems with my DD. Each time she goes to the doctor she gives a sample but it comes back as needing a repeat. I was talking to a friend who said it needed to be a mid-stream sample but we have never been told this.

She is now on an antibiotic every 8 hours, is drinking water regularly and going to the toilet more often. She has admitted to holding onto her urine for a long time when she is busy - could that cause the problem?

We have the same pattern. After 3 doses of antibiotic the pain goes away but once she stops antibiotics the pain comes back after 2-3 days. As the doctors have never had any results from her urine tests does that mean that the antibiotics she is given may not be the best ones available?

I am going back to the doctor on Tuesday. Should I insist on a scan? What will the scan be looking for?

Will get cranberry capsules tomorrow. We have tried cranberry juice but she does not like it very much.

saggarmakersbottomknocker · 22/07/2012 19:04

We went for capsules for the same reason silver.

Just a couple of points - the best sample is mid stream and taken first thing in the morning. Insist that it's tested at the lab and not just dipped to pin point the bug and the correct anti-bs, then as I said previously get a retest when she's finished the anti-biotics.

dd had an ultrasound and a scan with dye which I can't remember the name of (micturating something), basically to check the structure and whether there is any narrowing of the tubes or any backflow in the urine flow. There is another test too a DMSA scan which dd didn't have. Many GPs won't refer unless there have been repeated infections so you may have to go with getting the urine lab tested first and then go from there.

They can also put them on prophylactic antibiotics, usually low dose trimethoprim if they think that is necessary.

FedUpOfSlugs · 22/07/2012 20:50

Thanks you.

I think we will have to wait a couple of days now the antibiotics have stopped and get a sample properly tested at lab. Other samples have been lost or come back clear, so we haven't yet had any clear results of an infection other than from the dip tests. Have always done mid steam morning samples.

Worries me that although the pain is less when she is on most antibiotics it never goes away completely and she complains of tummy pain all the way through the course.

Will ask about scans as well. Thanks again.

Sorry if this is daft Advice but is she wiping front to back? Maybe she is scared of the uti so everytime she gets a twinge, she panics, gets anxious, wee's more? HTH and hopefully feels better soon

saggarmakersbottomknocker · 22/07/2012 21:17

FedUp - possibly interstitital cystitis? Though it's more common in older women.

FedUpOfSlugs · 23/07/2012 20:57

Yes, think she wipes correctly. I suppose interstitial cystitis is possible, not heard of it before.

Thanks

dd's UTI hasn't cleared up according to today's retest so she's on Co-Amoxiclav now. How's things with you?

silver73 · 28/07/2012 12:42

Hi my daughter finished her fourth course of antibiotics yesterday. She was put on Co-amoxkclav as well. This is the same UTI that antibiotics have not managed to shift. The series of urine tests have not come up with a definitive microbe to treat either. Thanks for the suggestion of Cranberry capsules GP agreed when I suggested it to her. She is also on probiotics.

We have another GP appointment on Monday morning and this will be 72 hours after finishing antibiotics so either the UTI will flare up again as it has every time DD stops taking antibiotics or if she has no symptoms I am still taking in a sample to ask for it to be tested to check the infection has gone. If it comes back I am going to insist on a scan.

DD was clear of bacteria in her last sample but on Saturday (ten days later) the pain came back albeit in a minor way and just when passing urine. DD then consistently started taking her cranberry capsules and today she had pain twice when passing urine but the time she did the sample for the doctor she was pain free. Can cranberry capsules help?

The doctor has referred her for a scan and sent the urine sample off again. DD has also been given a broad spectrum antibiotic to take if the pain gets bad. She felt her tummy and there is no tenderness.

I think it was one UTI that was not treated with the correct antibiotics because none of the samples came back with any clear indication of the microbe involved. Once the microbe was identified antibiotics worked.

I am worried that the infection is back. Doctor did not suggest that it could be anything else.

silver73 · 17/08/2012 01:29

DD had a scan of kidneys etc all functioning normally and no abnormality seen. She has been on antibiotics for the last week. Urine test has come back negative even though she still has discomfort when passing urine. Back to the GP tomorrow any ideas? I am worried that she has a low level infection when on antibiotics that springs back into life once she stops taking the antibiotics and that if this is not sorted out it could cause damage to her kidneys.

DameSaggarmakersbottomknocker · 07/09/2012 18:30

How's everyone getting on?

dd tested today and we're seemingly back to square one. Back on anti-bs for a couple of weeks and a referral for another scan. She has none of the usual symptoms (except she has a constant headache and feels crap).

dikkertjedap · 07/09/2012 19:27

Hope both your dds are getting better but if not I would insist on urgent referral to a specialist.
