assuming you have talked about this with them...

Dear xxx
as per our recent conversation, I am writing to confirm that childs name last day in my care will be xx xx. I would like to take this opportunity to wish you and childs name the best of luck in the future.

or something a bit like that?

talk first, may be you do not need letter at all.

I'd say always better to put in writing - even if you had spoken to them, otherwise you have no evidence of what was said/leaving dates etc.
I'd say something like:

Dear xxx,

I am writing to inform you that due to xxx I will unfortunately no longer be able to care for your child on xxx days.

As per our contract I am hereby giving you 4(?) weeks notice of my decision to terminate our contract and xxx's last day in my care will be xxx.

I have greatly enjoyed looking after xxx and wish them all the best in the future.

Yours sincerely,
