DS always comes back from CM with bright red nappy rash

He's in 2 days a week and every time, he comes back with bright red, very painful nappy rash (he screams whenever he even gets close to changing mat).

Any advice on what is the best way to deal with this?

I appreciate he will not get changed so quickly as when he is at home, I only look after him.

Plus, he does have really sensitive skin - if I've not been aware he's had a poo when I'm with him and his nappy has been inadvertently left on for a little while post-poo (so to speak!), his bum goes bright red. I guess I'm over-cautious, as I know he has such sensitive skin and he poos 2-3 times a day.

She's a fab childminder and I really don't want to offend her, but I don't want him as upset as he has been tonight and in so much discomfort. She is aware, as she mentioned today how sensitive his skin is and how his bum reacts to poo. So, I think she saw it before I picked him up.
Any words of advice really appreciated. I'm going to get some tea, but will log on later, thanks for reading.

Sorry, that looked a bit curt! I meant that following on from one of the other posters, maybe he is teething at the mo which on top of his sensitive skin is making for particular probs. This used to happen with my DS.

I agree with the other posters that you are well within your rights to ask her to do something about it and you're not being unreasonable.

your poor ds - nappy rash is so painful

does seem a bit weird that he suffers more at hers then yours,somaybe she isnt changing frequently

agree Metanium is fab, but def dont use all the time as otherwise it wont be as effective

smoother in vaseline after each change and try to allow to go nannyless sometimes after pooed to allow air to get to it

Please do not feel funny about asking your CM to do anything That's what she is there for! I believe in 100% open communication - of the good and the bad.

The above advise is great. I hope it settles soon.

thank you for all of your ideas. i will talk to her about it, now i've formed some ideas of ways to talk to her without saying "he always comes back from yours with a horrendous nappy rash, are you changing him enough".

I always changed mine every two hours plus extra in between if they had pooed. I used washable nappies though and zinc and castor oil cream at most changes.

IMO the CM is either not changing him enough (5 nappies from 6 came back?) or she is using wipes on him that aren't suiting him.

IMO CM should give as good care as mum would and if I was sending home a child with a sore bottom every time when I nannied I would be mortified. It just wouldn't happen.

It might be something to do with food but has he always been like this?

If he has then it's unlikely to be a food intolerance affection the make-up of his poo!

We had this at our CM a while back. I worked 3 days a week and by the third day the nappy rash was really bad, bright red and he was screaming and begging us not to change him. it was usually nearly clear by the time he had to go back again. it took us a few weeks to notice the pattern. We raised it with the childminders (husband and wife pair) but they said 'we always change him straight away'. He was in agony. When i went on maternity leave it all stopped as he was at home with me. I am dreading trying to find a new CM now i am going back to work soon. He's toilet trained now, but DD isn't. I'm sure lots of / most childminders are very good with changing nappies but i don't know how to guess which ones are.
Don't know what to suggest with OP though. Try asking, politely to change more often, even if your DC is just wet. And send a load more nappies there.

I just wanted to recommend sprilon spray for bad nappy rash, it's designed for bed sores, is pricey but works really well.

Just wanted to second the wipes thing - my DD1 was fine at the CM's, then started to get bad nappy rash. When I raised it with the CM, she was baffled, as she hadn't changed her routine. She then went and checked the brand of wipes she was using and discovered they had changed the composition of the wipes. Since then, she has bought a specific brand of wipes that we know are OK for my DDs, and always uses them for my 3.

I looooove my CM, she is never offended when I ask questions or ask her to change something.

when they are wet or soiled which varies from child to child and whether they have a sore bot or not!

thank you everyone for posting, it's great to hear everyones views and recommendations!
tbh, now i've had a chance to mill it over (and of course, ds bottom is now nearly back to normal), i feel more objective in asking my cm to use the nappies i provide and i've also bought some more gentle wipes now too and some cream. i know it'll have cleared by tuesday, when he goes back, so i'm going to say that he's been very sore recently and i'm just trying to make everything consistent (i.e. cream, nappies etc) to try to keep it under control, so i can work out what's causing it. i'm also stopping tomatoes for a week, as dh is convinced it's a day or so after he's had tomatoey based food.

his poo really is so runny, it fills the whole width of the nappy and about 6inches long or so. i.e. totally can't see any nappy in that area. i am going to see a doctor i think, as whilst it is normal for him and what i'm used to, it seems very odd he never has solid poos. as the cm keeps writing 'horrible nappies' in his little book, i wonder if she's trying to tell me something, without telling me iyswim.

unless the cm is using her own nannpies

i would be rather annoyed that you leave 6nappys and comes back with 5

that measnONE chnage through out the day

i think nurserys have to check and change every 2/3hrs

there is no hard/fast rule,but i would personally chnage every 2/3hrs and more if had sore bum - sp in an avergae day should get through at least 4nappies, more if pooed inbetween

she made a point of telling me on thursday that he had had two dirty nappies and wrote this in his book, so she is using other nappies from somewhere. and his pack of wipes never seems to go down either. so i think she must be using other stuff.

i am a natural born worrier though, it's in my blood. i am really pleased with how everything is going, she is really good (and i've had friends who've seen my ds out at playgroup who've told me how good and attentive she seemed to them). that's why i feel a bit guilty even posting this. i originally just wanted a bit of practical advice how to ask without saying "god he comes back very sore after being with you - are you changing him regularly enough?". but i know now how i'm going to ask on tuesday - thank you.

SMM I don't change DS anything like every 2 hours I assume you are talking about FBG's post, but I think she is a nanny (?)
DS is changed probably every 4 hours, or straight away if he's pooed of course. He also spends all night in one nappy (but never poos during the night). Hope that doesn't make me a bad mum!

I was a nanny but the mujs used disposables which could go longer.

I had a two hour rule with my kids as I was worried I wuold forget to change them otherwise!

Just to say that I spoke with CM yesterday, who has been brilliant. She's using cotton wool and water (she was doing this apparently anyway, which is why the wipes never seemed to be going down) and I turned up today to see my DS getting some nappy free time (running around in the garden with just a hat, t-shirt and his shoes on, pushing a pram, with his two little chubby cheeks showing as he walked along, so cute, anyway i digress!).

She's been fab and very accommodating (using the cream, checking frequently, suggesting using flannel things in a catalogue she showed me) and lovely about it all. Told me when she changed him and when it flared up a bit, how it's flared up a bit with just a bit of a wet nappy etc etc, really good and really working with me. I don't know what I was worried about. Yay!

pleased to hear you are reassured your CM is suppoting you and your litle boy. Its funny your DH mentions tomatoes as i used to mind a little boy who had the same reaction to them and so did my DD. I found it only happened wehn they were cooked as well, not raw

teething also affects the acidity of urine so is also a classic time for sore bottys
