andi just want to make sure that i have done it all corectlly. Have already meet parents in January. all very nice and will be looking after a 6 year old girl after school, its not every day, her parents are going to give me their rota 1 month in advacne and i can invoice all the hours, have agreed anything more/less will be added to the following month.

Mum is coming round tomorrow to sign paper work, do i have it all, i have
Permission forms for
: Outings/trips in my car
: medical treatemnt
: medicinces Prescribed and non prescribed
: Photos

have the NCMA contract ( filled out my bits already )
and the child record forms, is that it ??

is it the child record forms that i put in a sealed envolpe for a back up childminding

well done

get contract terms written up for her/him to sign too(i agree that i will give x notice of hours, i agree to minimum x hours per week etc)
Ask them for routine for child, even just verbal, what she likes/has to do after school etc

cant think much else right now, but someone else will be more help

thanks strawberry - all very exciting , glad to be statring of this one as im 17 weeks pg and this eases me into it.. Parent know im pg and the little girl is so happy that after school hols there will be a baby here , i have built in help

thats great, congratulations!..im sure it will all go well and if you do think of something later, you can alsways ask them to sign afterwards

I just have 1 consent form with everthing on it and if it doesn't apply to that child I cross it out.

Don't forget appling suncream, seeking emergency medical treatment or advice if needed, asthma inhalers or epi pen for anaphylactic shock.

I know you said about medical treatment but the last ofsted inspector said I needed to include advice.

Incase you need to phone the childs Dr. or NHS direct for advice about the child.

Yeah, I missed the bit mumlove is talking about but got it all up to date before my inspection thank god. It's the page right at the front of the medical, accident book.

Can't remember what's listed but this is what I have permission for myself (after advice from mumsnet, as Theprisoner said at the time, they want a form for everything if they are that picky and yes, probably even want a form for picking your own nose!!! ):

Outings
Suncream
Travelling in my car
Photos (with seperate tick boxes as some parents are happy with photo's but not on my website etc)
I've got one for teething gel (sod getting the parents to sign for that each time!!!!)
Playing on my garden toys (i.e. trampoline etc)
Pets (I have a cat so it just says that child can/cannot touch him, permission for child to be in room with him etc)

May have some others but I'm away from home at the moment so can't check.

Normally at my contract signings I have:

Contract (remember to list everything you want them to provide each day!! You'll be suprised how many don't provide change of clothes etc)
Child Record Form
The above permission forms
Get them to sign the red accident etc book

When I say THEY want permission forms for everything, I meant OFSTED and not the parents!
