Just that really. Is there a standard form I can use? Is it my responsibility (employer) or hers to identify any potential risk? Any obvious ones other than lifting?
Thanks!

I'm really not sure but I'd think it's your responsibility as her employer (the way it would be for most other employers and employees).

Other things to consider - being on her feet for too long, trip hazards and heavy lifting, maybe potential exposure to children with contagious infections? I'm just guessing really, sorry!

The risk assessment should be done as soon as practical after your nanny gives you their pregnancy news. You should ideally sit down with her and go through the assessment form and agree necessary action. If risks cannot be minimised then you would have to offer alternative work or garden leave. Also give her details if smp or ma are appropriate. The following link has a good explanatory assessment form downloadable but specific to a uk university but a good basis for you
