I also use the NCMA ones, but I sometimes add bits in the "additional notes" - such as term-time only contract, minimum one hour charge afterschool etc.

On the last sheet, where you have to write down anything parents should provide, make sure that you do write down stuff if they should bring it (eg. lunch, drinks)

I would definitely use the NCMA contracts to start with simply because it should ensure that you cover just about everything you'd want to. They also look very professional. I know quite a few minders that make their own once they are more experienced.

well ive been doing mine on puter, but its got to a point where i dont know where to stop and keep adding bits, it'll be like an ensiclopedia by time im done!
so do i have to wait until im reg to get them from ncma?..i was just trying to get stuff done now until i can become reg when move house?

If you join the NCMA, which I assume that you wouldn't be able to do if you are not yet registered (I could be wrong?), then you can buy stuff cheaper. I would think that you could buy stuff from them now, without being registered, but it would obviously cost a bit more.

Or try ebay - quite often people are selling off paperwork if leaving etc! Just put 'childminder' or 'childminding' in the search bar!
