Wording for CM/School/Parent Agreement?

Can anyone please help me write up some kind of agreement to be signed by the school, the mindee's parent, and the childminder (me), saying that I will collect mindee on certain days of the week, and that we all agree/etc?

Assuming it should have:

My name, contact details, reg no.
Child's name, contact details, etc
Parent's name, contact details, etc
School details
Days of week
etc

I'm not sure what I need - as mindee is my first after-schoolie.

Help?!

Erm I don't have an agreement with the school. I just fill out a normal contract with the parents and then parents inform school about the arrangements. I usually introduce myself to the teacher (if i don;t already know them) and that's it/

Great, I'm all for doing less paperwork. Someone had mentioned having an agreement so I thought maybe it was the done thing...

I agree with nicknacks, make sure the parents have told the school and they are expecting you, but in an emergency the school would call the parent and then the parent would call you if needed.

With Early Years children OFSTED like you to sign the children over to the school; but schools sometimes prefer not to

Thanks everyone. The child is not in the early years group. I will ask the parent to add my name to the list of people with permission to collect the child, rather than make my own permission document.

i've always had to write a letter to the school/preschool stating who was picking up if it wasn't me (not occasional pick up by friends), but longer standing arrangements like a childminder
