How does one 'breathe' the baby out?

Following DS3s birth I suffered horrific piles (thrombosed) due to pushing like a mad thing as I wanted it over with. "nd stage was 4 mins 30.
My previous 2 births were waterbirths but due to staff shortage I couldn't use the pool 3rd time round and found the whole 'dry-land' experience very strange - felt a bit exposed and on show.
I am terrified of giving birth again (in 6 weeks time) not because of the pain of labour etc so much but if I get those horrific piles back... I can't remember the first 2 weeks of DS3s life as I was in horrendous pain, couldn't really sit or stand, had to lie on my side so was house bound.
How do you resist the urge to push and just breathe the baby out - I am asuming this method will greatly reduce my chances of a repeat of last time?

Hypnobirthing breathing for birth breathing concentrates on the out breath.

Take a deep breath and work with the out breat - breathe down slowly using your imagination to send enefgy down and out through the birth path. Visualise the vagina opening liek the petals or a rosebud (teeth touching but not clenched, tounge lightly touching palate)
Bear down for approximately 10 secs before taking another deep breath and continue to work with your body as long as it is surging (hypnobirth speak for contraction.
As you get closer to crowning take a strong deep breatha nd firmly direct the out breath down and forward in a J pattern to follow the shape of the birth path. These breaths are firm, pusing J breaths to assist in birthing your baby

Give birth standing up. Baby will practically fall out, minimal effort required on your behalf.

definitely agree with do it standing up or squatting.......if this is your 4th baby he/she should just literally fall out!

Water birth made the most difference, I agree you feel less exposed and on show.

Is a home birth an option? That way you get your water birth as you provide your own pool.

Both DSs born at home, DS on dry land, DS2 in water, second birth was much easier and far less painful. No tearing despite either bad second degree tear first time round.

That is such a good question. Thanks for asking it LadyoftheFlowers, and thanks for answering Piglett. I've been listening to the hypnobirthing CDs and still don't really get it. I'm really hoping the midwife will explain on the day...

I am hoping to to this technique for ds2.
I am 39 + 2 now and have the worst piles , to date !
Am dreading pushing, have been Reading hypnobirthing and listening to mindful mama cd.
I have been practising with poos, but obviously these aren't the same size as a baby !
I think you are supposed to stay relaxed and keep breathing through the grunt pushes, not hold your breath and push, try to keep taking a breath, breath out and down, like the j thing, just don't hold and push !
That's what I am hoping will happen anyway, time will tell !

This all sounds very good - certainly hope she does kinda fall out herself - would be nice.

It dawned on me last night, that seeing as I don't have them now - being preg alone doesn't seem to give me piles - if I do get them this time they should appear pretty much straight away as I have had them before (they appeared in a shocking manner last time, 2 days after delivery) and this time I will know what they are and be able to push them back from whence they came. I thought I was losing intestine or something last time and by the time a Doc said 'Grab some lube and 'pop' them back in!' they had become thrombosed so wouldn't go.

Sorry for 'TMI' but this has relaxed me no end - I was so scared before I had this brainwave.

The joys of being a woman eh? And a pregnant one....
