Managed of physiological third stage?

I've been getting differing information from various people about this, and I'm having trouble making a decision.

I have no problem with having the injection for a managed third stage, but not until AFTER and the cord has stopped pulsating, is cut and the baby has been handed to me.

Apparently (sorry, I didn't know this) they inject you when the baby is half out, and cut the cord really quickly. I just wanted to delay the injection for a while.

My midwife has told me I can't have this. Either I have the injection immediately, or I have no injection at all. Apparently there is no point having the injection after the cord has stopped pulsating. This doesn't make any sense to me - surely it will still make the uterus contract and assist with the delivery of the placenta.

Can anybody help me/advise on this? I've read up on it, but can't find anything about waiting and having the injection, instead of the usual way of doing things.

The trouble is that she's put 'natural third stage' on my birth plan, which what I asked for.

Thanks very much.

Stefka...it's sad that women worry about "irritating" their midwives, sigh.....it really shouldn't be like this, afterall, it's your body, your Baby and your Birth , so, you hvae all the right in the world in making the decisions what should happen (obvioulsy I am not advocating that you should go against m/w advice at all costs, etc...)

That's not true! I have the same wishes as you and had this conversation with my consultant. I have bood clotting issues and therefore I need to have the jab to stop me from bleeding too much but the consultant said a compromise can be reached and the cord can be cut once it stops pulsing and then I can be given the shot. The timing of the shot is not an issue for you but it is for the baby. I read up on this but can't remember where, I'll dig out the article and come back and tell you where I found it

Sorry - just to be clear - I was referring to the MW's words in the OP! not anything subsequent!

Stefka, your link is not working correctly, I presume! It goes to a Mr Men and Persil site

Oh how we love pg brain!

3andnogore · 18/10/2007 12:10

Klaw...what link??????????????

Stefka · 18/10/2007 14:17

I don't know why that became a link - it wasn't meant to be one!

i had a physiological third stage with dd, but it was different to how other discribed it, the cord wasnt cut until after the placenta was delivered and i did have to push with guidancance from midwife as i had no real urge.

i had breastfed dd in the meantime, she was latched on within mins but placenta wasnt delivered for a while, i think it was coming up for an hour which is why i was encouraged to push. I was very happy at the time.

i then had lochia for 8 weeks minimum, was still bleeding at 8 week gp check, had scan to check for retained placenta nothing there but not suire how much longer i bled for. I had been considering having injection this time am 4 months pg but some people on here are suggesting it makes the lochia worse, could anyone shed some light on this.

Lulumama · 18/10/2007 17:07

not sure how the injection could make the bleeding worse..sure someone could explain

basically the lochia is bleeding from the site the placenta was, it is basically a wound. so the bigger the placenta, the bigger the area to bleed

lochia can last 6 weeks, without it being abnormal. as long as no massive clots, or bad smell, or fever

didn;t know you were expecting again ,congratulations !

With dd2 i had injection but cord broke and i was unable to push placenta out. It then ruptured and lost a litre of blood. They managed to get it out just as i was being prepared for theatre. I had lots of bleeding with small clots for about four weeks. With my first dd i had injection and bf and it came straight out.

barbamama · 18/10/2007 22:08

I definitely bled less heavily and for less time when I had a natural physiological 3rd stage - don't know if this is always the case. Having had one of each type, I would thoroughly recommend natural delivery of the placenta if the birth has gone well - mine came out with very little effort or pain a few minutes after I started bf the baby about half an hour after birth. I didnt really get an urge to push but was able to push it our myself when the mw reminded me that I still needed to do so (was a bit wrapped up in the baby)

3andnogore · 19/10/2007 10:42

lol...still don't see a link....ponder....

Klaw · 19/10/2007 22:31

3andnogore, in Stefka's post she said "I only found it from reading about it here." And the 'here' was underlined and a different colour on my screen so I assumed she'd linked to a thread or another site so, me being nosey, I went to look! But it must have been a glinch on my screen, unless someone else saw it too.

Don't worry about it anymore
