You poor love, I empthize! I have had them quite badly this pregnancy (and am only at 27 weeks) and they can sometimes get bit worse postpartum. Not always, though - yes, its probably due to pelvic pressure generally, and enlarged blood vessels there generally, and the extra weight etc.
I also ended up with a section last time, and first poo ditto - urgh. All I can recommend is buying a large bottle of Lactulose in case fruit not enough after your section (pain meds cause constipation), and also buying some Anusol with hydrocortisone. My gp said it was fine, and the one without HC does absolutely nothing for mine - the one with HC does seem to soothe and shrink.
Mine are usually internal, and - sorry if TMI - the best tip I've ever ever had from my GP was to try and gently shove any that decided to pop out back in, whilst under the shower (ideally) because when in they get less inflamed, and obviously hurt a lot less.

Oh - if you can't get an external one to go back to being an internal one, try an ice pack (ie sit on a bag of frozen peas wrapped in teatowel or something), lie on your side not back to sleep, don't stand around if you can help it, and use one of those rubber ring things to sit on if you have one!

Have you used anything on it?
I'd go to your GP and get some cream or pessaries (or both)so you can try and get it cleared up before the birth.

suppositories pessaries really wouldn't help!

Just a message to all those suffering with piles - i am a midwife and I always recommend witch hazel gel - applied straight onto the piles for instant relief - have been told it works wonders. Also cypress oil in the bath or oral calcium flouride salts. Hope this helps.

I had minor piles during pregnancy which have now worsened considerably after labour (last week)and are now mainly external and quite painful. I'm using a pile cushion which really helps and I'm going to try this witch hazel gel mentioned by Rachel too. I have found that salt baths are very soothing for everything down there, at least for a few hours. Just a normal hot / warm bath with regular kitchen salt diluted - makes the water softer...

Norms - much sympathy - I've got same problem as you, had with DS too - just left them to get better after pg finished, took a few months and suppositries from docs.

If bleeding is it worth going to docs tomorrow or to see paharmacist?
