Sorry more questions for mears/other midwives - need advice though!

hi mears sorry to be asking you all this but am desperately in need of some advice. I'm 10 days overdue today with second baby (v straightforward pregnancy apart from some SPD pain) DD was 37+5 weeks and a 4h 20 min labour kicked off with waters breaking. Am booked in for a homebirth but this one is refusing to be born. My questions are:

  • how common is it to need an induction for a second baby when you have had one naturally the first time? Part of me is starting to think something could be wrong
  • am booked in for an induction on Saturday but this would mean that DP would be unable to be at the birth as he needs to look after DD, his SIL is away at the weekend but will be available Monday to looks after DD, is it dangerous to tell them I will wait another 48 hours before indcution, am I putting the baby at unnecessary risk?
  • had a sweep on Friday to no avail but was told my cervix was in the middle (neither anterior or posterior) and I was 1-2 cms (normal for second time mum) have also had loads of jelly like discharge last night but not blood stained but have less BHs and pressure down below then I did at 38 weeks/
  • if I do go in for induction next weekend will they just break my waters and wait and see, would hate to be strapped to the bed on monitors and drips and would hate to be in hospital overnight.

You poormthing, I'm afraid I can't offer any proper advice but I just wanted to say I'm keeping everyhting crossed that things will get started soon! Both my babies were at least a week overdue, although in both cases the threat of induction day spurred them on!

SK IMO the jelly like discharge is a good thing hang on in there hon, hopefully you'll kick off today, in the meantime bump!

SK - I'm sorry to hear you are still waiting . Although I am not an expert, I have had over due babies induced.

I had my first induced, my second came on his own (after an overdose on Lucozade ) and then my third was also late and induced. So it is possible to have a natural and then an induction.
I should think if you are closly monitered it would be fine to wait a little while til DH is available to be with you and there is someone to have DD. As long as things were stable than I can't see why not.
I was induced at 41+5 with both my inductions. I had prostaglandin gel both times, and I was able to move around. I wasn't monitored constantly, just periodically with hand held doppler. Obviously it depends on your hospital, but if you request as normal a delivery as possible they will obligue.

I really do hope it is soon and everything oges to plan. Not long now, sweetkitty - hang on in there sending you lots of good birth vibes xxxxx

sweetkitty, when I had a show, 24 hours before ds was born, it was completely clear - I was in hospital at the time and the mw saw it and said it was definitely a show, despite being clear.

I've just posted this on the due in Jan thread but I thought I'd put it here as well in case it helps anyone else.

Sweetkitty ? what can I say? Maybe you?ve gone today ? I hope so! If anyone knows what you?re going through, believe me, I do! Finn was born eventually at 40+14 and I am now an official expert on induction and cervical favourability after 7 attempts with prostin gel and an ARM/syntocin drip. If you want to ask me anything at all, please do ? or CAT me?

I think you would be fine to wait a bit longer ? my hospital policy is 40+13 if no other problems. You would give yourself the weekend to go naturally, plus you might become more favourable for induction. Remember I went home twice, once 10 days over and again 13 days over ? if I hadn?t had the section the next day both me and baby would have had a couple of hours monitoring on the antenatal day unit. Personally I was happy as long as he was moving loads. I?m not very good at links but type ?bishops score induction? into Google and you will get loads of info ? basically it is a pelvic score relating to engagement of head, length/dilation/effacement/softness etc of cervix ? it need to be anterior rather than posterior - ask your mw/drs what they have given you every time they examine you ? it should be 9 to be favourable for induction ? mine never got past 4 with the gel! If you can get to 6/7 you can have your waters broken and a syntocin drip put up (you need to be a little dilated/baby's head down for them to reach the waters safely - although despite this the senior reg did it for me on the labour ward at 1 cm - did I need a lot of gas n air!). Obviously these policies might be different for your hospital but it gives you an idea. After 6 gels, any further decisions had to be made at consultant level. Remember though, I am a medical freak! It is very rare for a multipara with previous normal deliveries to have failed induction.

With ds1 my waters broke at 10-11 days over and I went into labour a day later ? no induction.
With ds2 I was 13 days over, had several prostins over a couple of days which worked.
With dd I was 12 days over, had ONE prostin and went to 5cm in an hour!!!

With ds3 ? well read the birth announcement!

So who knows?

Advice to cope ? don?t get too excited at any twinges ? you get false prostin pains that do bugger all in terms of dilating your cervix. You?ll know when you?re in real labour. (beware, the prostin stings your bits rather, but it does pass, they virtually do a sweep every time they examine you).

Save your energy ? what will be, will be, relax ? it could take more than a day?.or two?! They will monitor you and baby before and after every gel (also assess your cervix) and will be cautious not to over-stimulate your uterus.

My first turned up the day after he was due. The second arrived 12 days late after 4 sweeps and me doing a lot of bouncing up and down (interesting at 9 months preg) so no need to assume anything is wrong just because you are overdue. I think you can ask for a scan to check on the placenta and if everything is still looking ok you can delay the induction by a few days.

Am just getting ready to leave house but just to reassure you sweetkitty, it is not unusual for second pregnancies to last longer than first ones.

The requirement for induction depends on the hospital threshhold and how the mum feeld.

In our area we would support a woman going past 42 weeks if she wanted, especially with thoughts of homebirth. You could let the hospital know your childcare predicamant and I am sure they would accomodate youif all is well with your pregnancy. They might want to monitoer pregnancy/do a scan. What if you labour spontaneously on Saturday at 42 weeks - will you no longer have homebirth?

Usually labour is started by giving prostin gel (or tablets). The waters are broken when the cervix is open enough so sometimes prostin is not needed. If contractions do not start within 1-2 hours of waters being broken, then drip is started.

You do not need to be in bed with drip but you do need monitored. I have monitored women sitting on gym ball.
