can anyone help me understand labour notes?

I would be really grateful if anyone could help with the 'translation' of some terms in my labour notes. My first labour / birth was not a pleasant experience (I know - it isn't usually!!), and it took a long time to recover.
We're now expecting our second child, and although my midwife says that there is nothing in my first birth experience that warrants anything other than midwife-led care and a vaginal delivery, I'm still concerned. After my first delivery, I was told by the consultant who discharged me that I could try for a VB, but that a CS would also be offered and discussed, and a decision made as to the best course of action. Now, I'm dealing with a MW and a different story. The MW is offering / encouraging a home birth, which seems a mile away from the suggested offer of a CS. I don't know what to do, but I'm wondering if there is anything in the notes that might help make a decision

So here's what I have:
I know, from what was said to me after the birth, that I had a forceps delivery, an episiotomy, a third degree tear, and a PPH of about 1300mls. I had, and still have, some continence issues, and a prolapse (much better now after physio) and a cystocele (still there).

What I've found in the hospital notes confirms the above, but with a bit more detail.
Baby is described, when we were told we needed assisted delivery, as deflexed OP, caput 2+, pp at spines / positive station, (an earlier entry says pp -1 to spines, so I assume this means that I managed to get somewhere with pushing??)
I had a third degree tear, which was stitched in theatre. Notes also say 'episiotomy extended to ????? (can't read this word at all!) and Ischiorectal fossa.' The word I can't read looks like 'funo' or 'fino', but could be anything! I've no idea where my ischiorectal fossa is (other than I think I could feel stitches somewhere unlikely for a while afterwards) Is this anything unusual?
Next line details the stitching, and the line after describes a right lateral vaginal tear. I can work that out.
Next section talks about PPH, attributed to 'atonic uterus'. Is this a function of the delivery, or does it mean that there is some issue with my uterus that is likely to recur in a subsequent delivery? I've done some googling, and think the former, but if anyone has any experience I'd be grateful.

Thank you in advance for any information / advice you have. I've been reading various threads here about the tearing / CS debate, so I'm clearly not alone in this. I don't even know if I'll get offered an ELCS (or if I would take it), but if there is something in my previous experience that would help to make the decision, or make it easier, that would really help. On the bright side, I've met, in the form of my labour notes, someone with worse handwriting than me Wink

Thanks Loopymumsy! I'm seeing the MW soon, so will raise again the consultant issue with her, more assertively. She suggested counselling for PTSD last time.

First labour was a pretty slow process. Started with show and contractions every 5-6 minutes and stayed at home, although phoned the hospital a couple of times; had some red blood loss after about 30 hrs and told to go in to hospital for checking as contractions had stopped. Discovered I was 4cm dilated, and contractions restarted, noted as '2 in 10'. Stayed in labour unit.
Five hours from here, had reached 5cm and went for ARM.
Four hours after this, had reached 7-8cm.
Three hours after this, had reached 8-9cm, and decision made to discuss with registrar.
Two hours later, registrar decided either 9cm or fully dilated (notes suggest I was finding examination too painful for her to get a clear sense of action), contractions described as 3-4 in 10. Recommended epidural and syntocin.
An hour later, baby had moved from -1 to 'at spines', but after another hour of pushing, no progress, so moved to forceps.

Does that make things any clearer? It does sound as though I was pretty slow to get anywhere really! Maybe no.2 will be quicker Grin
