Emergency c/s last time, what would be my options next time?

After 36 hour labour ds was back to back, and got stuck, so I had an emergency c/s.

Would I be able to choose a c/s this time? What about using a biryhring pool if I wanted to have a vbac? Would I be able to have an active birth or would I be continuously monitored?

Is there a greater risk of head getting a stuck again (have quite a small frame), resulting in another emcs?

hi geordieminx - i can't answer any of your questions but i'm popping in to say hi because i want to know myself. i had an emcs after ropey heartbeat during induction. it was awful and dh and i are determined number 2 won't enter the world that way! hope someone will be along soon x

geordie of course you can try for a vbac if thats what you want, as you can too suntangirl. Noone can tell you you're "not allowed" they can only advise and give you the odds etc.

however they can't know if what happened before is going to happen again. Did you have an epidural for instance? (long labour) as that may have made it tricky for you to use good positions to move your baby from babk to back and into a good position for birth

Back to back does not always mean long, painful labour, it scares people into believing it does as we only hear the negative stories about it. Don't worry too much about position like that as such

Did you have waters broken for you as that can cause head to come down before nature would have intended sometimes and therefore baby isn't quite ready to be born but has to be, (does that make sense? ) ie if nature had taken it's course, maybe baby would have decsended at a more regular pace and therefore presented in a more birth friendly position.

You can use a pool/and or hypno as much as you like, you may have to get signed off by cons to use one at hosp but you can insist, they can use delaying tactics but can't deny you.

stg it's difficult to know what caused distress in your baby but could have been any number of things that just follwed on from each other, often happens, doesn't mean it's going to happen again.

In both your cases, might be worth going through your last birth notes from the labour and seeing for yourselves where things started to change in the direction on intervention, you might see th pattern.

Geordieminx- ds1 was long labour Cos back to back and like you EMCS. Assumed ds2 would have to be section too but consultant gave me the choice. Had successful vbac with ds2 in 7 hours. Went on to have successful waterbirth vbac with ds3 in 4 hours. Vbac great - much better than emcs especially in terms of recovery. Waterbirth mixed with a bit of hypnobirthing was fab. Just get baby in a good position for labour.

I had a e/c-section with my first, he was also back to back, unfortunatly they didnt realise this till it was too late he got stuck, they tried for a long time with a ventouse then finally gave in and done a c-section. When I went to discuss what would happen with my 2nd pregnancy they said there was no reason to try for a normal delivery. I did have normal delivery with dd but I did need help with a ventouse again, the doctor said I was very small thats why I had problems.
They would let you try for a vbac if thats what you wanted, Im not sure if you could opt for a c-section, not sure what the rules are tbh

lia66 I did have an epidural, and everything just about stopped after that Sad although I don't think I could have coped with the pain much more at that point as I had been labouring for 24 hours. Interesting point about my waters, honestly, I can't actually remember them breaking, certainly the contractions started first, I'm not sure whether they ever actually went. I got to 7 cm I think when they took mr to theatre. Don't get mr wrong though, after 36hours I was delighted to have a c/s!

Geordie, I had a stuck DD1 and tried for a VBAC in water at my local hospital with DD2. It didn't work out (she got stuck in the same position - this is, apparently, very unusual) but the labour was infinitely better.

For example, with DD1 I had an epidural at 4cm because I was stuck on a bed on the monitor, in pain with a bitch of a MW who wouldn't let me get up (and who only appeared once an hour, did an internal, tutted and me and vanished). It was hellish. With DD2 I laboured at home, very relaxed etc, when it got interesting I went to the hospital, arrived there at 10cm, got in the pool and my body started pushing. Unfortunately at this point the baby got distressed so I went for a crash section, but the point is that the labour was way better, less painful etc than the first time around.

Now if I have a third I will have an elcs (2 having got stuck in the same way suggests I am a dud), but I certainly don't regret giving the VBAC a try because I know that if my pelvis worked I would have managed to get DD2 out and would have stayed pretty much in control.

I guess the point is you have options. I don't know anyone who has been denied an elcs after an emcs, and my doctor at least was very supportive of my WBAC attempt. No matter what you choose it will be a world away from your first experience.

i also had a back to back, stuck baby resulting in emcs.

i booked myself an elcs at about 34wks with dd but waited to see what her position was nearer the time before going ahead with it(you can back out once booked) she was still back to back at 37wks so i went ahead with with elcs rather than risk her getting stuck like ds did.

Hi I can only speak from personal experience but for DS1 I got to 9 cm when his heart rate dropped dramatically so I was rushed in for a crash ces.

When it came to DS2 the hospital couldn't have been more helpful. I was able to discuss all worries and concerns with them. We decided to go for e/s week prior on the understanding that if I started naturally they would work with me to have a natural birth.

The reason we chose this way (you can think me barmy if you like Wink) was from the moment we realised I was pregnant I knew he would arrive 2 weeks early from due date given, and that feeling never left me Hmm. My thinking was the best of both worlds without the worry if I did go full term that I would be rushed in theatre again.

Exactly 2 weeks prior at 6.30am my waters broke and DS2 arrived 11 hours later naturally Grin.

I had a fantastic midwife who knew of my fear of needles and knew that if they had put a drip/canula in I would have been a mess. I was told that because I had had a crash ces before as soon as I was admitted they should have put a canula line in just in case I had to have another but she fought my corner. It may also have had something to do with telling the Dr if she came near me with the needle I would punch her lights out Blush I blame the gas and air not normally aggressive.

Sorry if I have gone on but it can be done just make sure you talk everything through however small your query is.

you definately have options. I found my consultant not supportive of a waterbirth for VBAC but I went to see consultant MW at the hospital (just called for an appointment - nothing formal) and she was really supportive. Said loads of women do it and she called my consultant to say that was what I wanted and she was going to support me. Watrebirth was honestly brilliant compared the the 50hour B2B labour strapped to a bed with DS1. No CFM with it - they just listened in every 20 mins. There are lots of other signs an experience MW can spot to tell if your baby is getting distressed (apparently).

Anyway was given all the chat that I needed CFM with DS2 VBAC. Got to hospital in labour and they told me they were too busy to deal with me and I had to wait in reception. Got to 8cm in reception with no help or pain relief and the staff were too busy to really help - they just told me to stop being so noisy cos I was disturbing the other people waiting.

as others have said depends quite a lot on the indivduals involved but worth looking up the NICE guidelines if you meet with resistance. My HCP isn't keen on water birth VBACS becuase they seem to want to constantly monitor EVERYONE. I decided on an ELCS after a disatrous and failed induction. They wn;t let me go overdue because I have GD so I have said that unless I go ito spontanous labour on DD I will have CS thanks! I got the consultant to sign off on it last week but you DO have to negotiate a bit.

Re the continuous monitoring, they cannot force you to have it with any birth (incl a VBAC). Once I had made it clear that I would not consent to CFM unless it was indicated, the waterbirth part became easy to accomodate. I agreed instead to intermittent monitoring and then monitoring with a handheld after every contraction in the secodn stage. This picked up the baby's problems and then I got out of the pool for continuous monitoring and EMCS.

You need to read up on the risks v benefits of CFM. "The VBAC Handbook" is a good place to start.

my labour with ds went very wrong, massive amount of bleeding, ds in distress and went for an em csection.

I was given the option of having a section this time around and went for it, although they gave me the option of vbac with constant monitoring.

I have a good chance of giving birth naturally but with everything that happened (they really fluffed things up before i got to hosp) i decided it would be better for me mentally to go for the elc

im 34 weeks, and have just spoken to the consultant to confirm this.

be warned though, m/w cant tell you what they think you should do. you have to make this decision on your own. i asked for advice and they wouldn't give any. i suppose they could face repercussions if they advised one way and it went wrong :(
