Where to start with assessment for teen?

14yo dds behaviour is getting out of control.

Things I’ve noticed-

Room is an absolute tip and she doesn’t care
Constantly hiding food and eating it upstairs
Swearing at me and calling me names in front of younger siblings
Used to have ticks which have stopped now - Her eye would twitch
Temper will turn on instantly for a minor reason
Well behaved at school and her dads house
Only plays up at home

Does this sound like something health related? My older dd was nothing like this.

If so, where do I start? Gp or school? Thank you x

Unfortunately much of this sounds behavioural but that's not to say she isn't also struggling with her mental health. Eye twitch might be due to tiredness.
I would start with school and look at pastoral support, explain you are worried about her and her behaviour and it might be useful for her to have someone to check in with.
Ask her about whether she would like to speak to GP and access support, have a conversation if possible at a calm moment to say you are concerned and want to help.

I would say a lot of this sounds like teen behaviour ( I’ve had 3 teens so far ) but If you have concerns you have to go with your gut feeling, my daughter has just had a mental health assessment and it was noted signs of asd / adhd aswell as depression and mental health things so you can never be to careful, it got to the point that I had to remove her from school for her own wellbeing, the steps we took to get her a mental health assessment was a referral from the gp to camhs which then asked for more information then she got accepted on to there waiting list and all of a sudden out of the blue camhs sent a letter saying there working with healios to get wait times down and she had her assessment last week after only waiting a few month, they are doing 8 sessions to help with anxiety and confidence and she’s been referred back to camhs to see if they want them to do a asd / adhd assessment. Even if you call camh first point of access yourself they are really helpful, but definitely go with what you feel your child needs and push for them to be seen.

GP. Typical teen. Could track for pattern, cycle related. Give multivits and try to keep up dialogue.
