Sent home from hospital

My 88 DF has been hospital for 7 weeks after having a fall & low blood pressure. Unfortunately he is no better & effectively bed bound unless 2 people can help get him into chair. Currently has cathater etc. The hospital want to send him home - I assume to free up bed - and claim he will have some care but I am unclear how this works. My 86 DM uses a stick & can look after herself but no way can look after him, unfortunately my DS & I both live couple hourse away. DM is really worried they r going to send him home with no help.
Does he need a Care Needs Assesment - does the hospital arrange or do we need to request. Does level of care depend on needs? Also how quickly does everything happen as we will have to sort room downstairs for bed etc? They are South West if relevant.
Finally I assume they will have to pay as they have a house & some savings.

Reablement is for 6 weeks and is free. After that there is a financial assessment to see what contribution should be made for further care. In Wales it is capped at £ 100 per week and that is the same whether you have 2 cares 4 times a day or 1 carer once.

Can you speak to the discharge nurse on his ward and explain your concerns? Ask for an assessment from OT to see what your father needs at home. This didn't happen with my mum and we had a terrible struggle to get her on and off the toilet - and even to the bathroom.

The thing to stress is that to send him home without assessment and support will be an unsafe discharge.

It was not my experience when my elderly mother was discharged but I think that was partly to do with the onset of her dementia and her ability to pull the wool over the professionals' eyes! Bringing her home from hospital was more traumatic than saying goodbye as they wheeled her off to surgery with a 5% chance of survival!

We've recently had DFIL at home but we've had to fight for everything Flowers
