Can I please have a big pat on the back for getting this far?

Almost 3 weeks in and I am still breastfeeding my PFB! In fact as I type my beautiful baby is on the boob!

This may not sound like an achievement to many but let me explain some of the hurdles....

  1. Breast reduction 9 years ago (large reduction 7lbs taken out- one nipple doesnt function so well)- not even sure if I can breastfeed until I tried.

  2. Flat nipples so baby can't latch on without the disovery of fabulous nipple shields (genetic issue but not helped by reduction)

  3. Traumatic delivery and baby in NICU for first 5 days- no cuddles for first 24 hours of babies life, limited skin to skin- tricky with all tubes etc. Baby was oxygen starved (possible damage) and initially showed poor suck reflex but that improved within a day or so.

  4. Baby being tube fed my EBM every hour means baby full and has no interest in latching- keeps falling asleep at the boob- i have to maintain these top ups but once they are reduced to three hourly I can sometimes also get her latched for a feed. Except I also still have to pump and invariably she gets hungry just after I have expressed and there is no more milk! Breastfeeding support at hospital limited (lactation consultant promised everyday and never materialised) had help from speech and language therapist with latch though which was fantastic.

  5. Baby has to have MRI, she is sedated with suppositories she keeps pooing out (effectively enemas)- she ends up going without a feed for 6 hours due to the sedation, I pump in meantime but this mucks up the baby hungry, mums boobs full thing again plus difficult to get her feeding due to sedation.

  6. Baby loses 14% of bodyweight (not helped my mri enemas etc). Sent to hospital for dehydration check- supoosed to take an hour- takes 7 (and results come back fine). Mum doesnt have enough to eat or drink and feeding is tricky in hospital as doesn't have pillow etc, and its not home.

  7. Baby put on intense regime of feeds- Every 3 hours -feed at breast, give 40ml top up, then pump next feeds top up then sterilise everything for next feed. We have been doing this for a week now.

  8. Baby is slow to gain weight but is weeing pooing fine and feeding really really well. Midwifes keeping an eye but happy for the moment- next weight sunday. We sometimes have to give a 30ml formula top up if I dont have enough expressed or in boobs but its getting rarer as my boobs and baby are getting better synced (considering we are effectively a "week behind" on our breastfeeding establishment due to time in hospital then this is fine.

So yeah am just so so proud we are still feeding despite all these massive hurdles- and to think I only thought the breast reduction was going to be the issue (hadnt even realised about the flat nipples latch issue!).

So pats on the back would be nice to help inspire me to keep going because its really flipping hard sometimes! (once she has regained birthweight our feeding regime can ease off a bit- but until then its a bit mad! Oh but midwifes now suspect anyhow her brithweight (9.5lbs hence traumatic delivery I couldnt get her out) may have been inflated from all the drips i was on during labour- as she was a very very puffy baby when she came out and within 12 hours looked much more normal- trouble is the paeds wont believe that and will keep trying to admit us if her weight doesn't go up satisfactorily. Bah! We refused to be admitted last time as we have had a terrible time of it in hospital and are keen to avoid them as they dont help with getting her going on the boob!

Thandeka I have been wondering how you have been getting on How are things now after the rough time you have been having?

Well done for breast feeding. Dd is nearly 4 weeks old and Im still doing it as well. I thought I was doing well but when I read what you have been through!

What a nightmare about her weight. I would refuse to be admitted as well. Hospitals arent great at the best of times but I find that I fed so much better once I got out. I had no help except one mw ramming my boob into dds mouth. I ended up using the Bounty book pictures to get her latched on

I am expressing some so that I dont have to give formula if out or if dp wants to feed her. I am considering buying a 2nd pump so that I dont have to keep washing the one I have IYSWIM.

How is your dd? Hasnt the time gone fast?

Hey mama2moo glad bounty book pics saw you right!

I just brought extra pump bottles from amazon and luckily we have a spare pump kit from hospital which seems to also fit the medela so will help not having to wash it all the time!

My dd is conked out after a marathon feed- she is doing well and so far seems a perfectly normal baby- no evidence of this "damage" so thats good and fingers crossed their never is. The infant brain is very plastic so hopefully neurones and synapses will rewire around it!

if it helps at all, you don;t have to sterilise the pump every time - do it once a day and just stick it in the fridge in between

Awesome tip cheers mrs badger hope our fridge is clean enough! Although I have a section of fridge solely for ebm which is boxed in and hopefully spotless!

When I say sterilising I mean hot soapy water wash and rinse well and air dry. If it's good enough for neonatal intensive care it's good enough for me! But I do steam or boil sterilse now and again just as an additional measure.

Blimey two more hurdles occured to me last night (might as well "milk" this praise a bit more! Pun intended!)

  1. by day 7 breastfeeding was still a massive struggle as we had to always give her nasogastric tube 32ml top up every three hours and it was always at a time which caused her to fall asleep and not be awake for a feed. We hated that nasal tube and so did she - used to pull on it during breast feeds and would look uncomfy during top ups. So in the end we had to say no more tube feeds and do her top ups with a syringe in mouth (easyish) or cup (good skill to learn but spilt everywhere and resulted in very windy baby). This was kind of against medical advice as they wanted to keep tube feeds going but we persevered and as a result breastfeeding was able robe established must better basically because we stopped trying to work to nasal tube feed times and did three hourly breastfeeds plus top ups instead. The doctors were all freaking out and saying she is a big babyand you won't sustain he'd etc but it's what we had to do. With hindsight I think if morebreastfeeding support had Bern available at hospital we would have figured out Howe to sync up the tube and breastfeeds but we aren't medical people so were just following instructions until they really stopped working for us but is scary to go against medical advice! Anyhow of her 14%weightloss themajority of that was in hospital and after her enema. She did still lose a tiny bit more once we started this but it wasn't massive and she has gainedsince we did it so it was the right but scary decision for us

  2. (just so I can make it 10 hurdles!) for first week of her life she had a canula in her arm for her antibiotics. This made feeding on the right breast very difficult as this sore arm would get in way and hurt her if positioned wrong and she didn't seem to feed as well in the rugby ball hold.

So yeah extra proud now!

Sorry just had to come back to this thread and say- now 4 weeks in and am able to do at least half dd's feeds without the nipple shield- aiming to do all of them without it by 6 weeks.

Have also hired hospital grade breast pump to get better yield of EBM and lots of lactogenic herbs so am hoping shortly to get enough to remove formula top ups and give her solely boob and EBM. Am still very proud of overcoming the hurdles and now I even have mini goals- go me!

She gets weighed again today so lets hope for an increase again.....
