Help -still in pain - is this really still thrush plus Raynaud's/nipple vasospasm? (sorry long)

I have an EBF 8 wk DD (my first) and have really struggled with breastfeeding pain from the beginning. First, presumably due to a bad latch (despite MWs, HVs, etc. saying it was fine), I had badly cracked and bleeding nipples. They were excrutiatingly painful, but I slathered on the lanolin and persevered.

The cracks gradually healed after a few weeks as DD grew and we improved positions and my confidence, but the pain continued - particularly intense at beginning of feed, but continuous throughout in a burning kind of way. Pain then spread into my breasts after feeding, also a burning feeling, and would last for ages.

I, a MW and the GP all agreed this must be thrush resulting from the original nipple damage despite no white spots on me or DD. For the last 3+ weeks, DD has been on Nystatn and I've been on Diflucan/Flucozanole (400 mg loading dose, 200mg daily thereafter) and have done almost all of the other things recommended on MN including grapefruit seed extract, eliminating sugars, etc. from my diet, taking garlic and acidophilus supplements, changing nursing pads frequently, washing clothes at high temp, with vinegar, etc. Nipples seem marginally better, but still extremely painful at start of feed and continuing (especially on the right side) at a lower intensity for most of the feed, boobs still sore afterwards, although at least on the left I think this is a little bit less. Sometimes between feeds, my boobs (but not nipples) feel kind of itchy from the inside.

Other thing that is happening, especially on right, is that after a feed (but not immediately - nipple looks fine coming out of DD's mouth), on and off for an hour or more, the nipple and/or the areola blanches - i.e. goes completely white and very painful for a few seconds before the blood comes back. While this is happening, my whole breast radiates a burning pain, out and away from the nipple. I've read raynaud's in the nipple can be result of thrush and was hoping they'd both go away with the above treatment, but this doesn't seem to be working. Was I wrong about it being thrush? Would raynaud's alone cause pain beyond the nipple? I'm taking B6 and magnesium/calcium supplements, but what else can I do about it? It's actually the worst part now.

Should mention that all through this, DD is getting loads of milk, very content and healthy and has been charging up the centiles with loads of wet/dirty nappies in her wake.

I have no intention of stopping before 6mos and would ideally BF at least a year. I could continue with the level of pain that I currently have, but would really rather not and daydream about the time when I'm not constantly thinking about treating/drying/not letting things touch, etc. my nipples or being conscious of my boobs 24/7 and just get on with it all. Any and all advice would be hugely welcome!

oh really hope someone answers you as i have the same problem
white nipple and burning sensation for ages afterwards
was told it wasnt thrush as no signs in babies mouth

Oh gosh you poor thing. I have no advice except that a BF counsellor might have more answers - have you spoken to someone on one of the helplines or at a Baby Cafe?

Unfortunately I don't have a solution but I had thrush for 4 weeks and it was a nightmare to get rid of, The 2 things that I think finally cracked it were using a dilute solution of vinegar on my nipples as well as daktarin after every feed and ironing my bras. Once it was nearly gone I had the same thing, blanching after feeding but also randomly throughout the day with deep breast pain. I checked how dd was latching and made some minor adjustments (feeding through the pain for so long had meant I was pulling away a little bit) as I thought it may be due to compression. However it may have also have been due to the flucanazole (I have read conflicting thoughts on this though...) The blanching stopped at the same time as I stopped taking the flucanazole and the thrush finally fully cleared. Hope the same thing happens for you!

Sorry I don't have an answer but just wanted to say I experienced very similar thing (intense pain, blanching, possibility of thrush but none of the other symptoms) and I never got it sorted out but it did seem to sort itself out eventually. I'm not suggesting you should ignore it as I'm sure a good BF counsellor will be able to help you but I just wanted to reassure you that my problems did subside and I went on to BF for 12 months (just stopped last week )

Good luck getting it sorted.

I really feel for you as i had similar problems with thrush and deep breast pain and know just how painful and demoralising it can be. I just wanted to suggest going along to a local breast feeding support group, I found the advice and comfort invaluable, there were people there who had been through similar and other difficulties and trained bf councillors to help with latch and offer advice. Just being with other women for whom breast feeding was important was really reassuring.

Ultimately the thrush cleared up with treatment eventually and although the pain continued for a while it did eventually ease off and now am completely pain free.It sounds like you are doing all the right things I Just wanted you to know that things will get better.

Thanks everyone, hopefully I'll have a similar experience with it subsiding on its own and will continue to watch the latch and treating the nipples, etc. after every feed.

Anchovies I've also read that flucozanole could be linked to the blanching and stopped the pills yesterday so am hoping it does improve...

I have been going to a great BF support group and will contact the helplines again if things don't improve soon.

this sounds like you could have mastitis, i have just recently gone through all of the things that are described above, after repeated trips to my GP and various creams, lotions and even antibiotics,all for thrush, i took advice of mumsnetters (thankyou x) and went to a breastfeeding counciller, who wrote a letter to my GP telling them that i needed Daktarin cream and Novogel circular wound dressings for my nipples, they were so cracked and sore i was terfully dreading every feed,(and on the verge of giving up breast feeding), the dressings were fantastic, they healed my breasts in just days and were really easy to use as they are coated with a gel that seals the dressing to the nipple, she said that although there were some signs of thrush, the pain i was describing fitted more to the symptoms of mastitis, also i had redness around the side of my breasts, which showed infection as a result of prolonged cracked nipples, the thrush had gotten inside and caused infection in the milk ducts, my DS also had tongue tie, which had not been spotted by the HV, but was picked up straight away by the breast feeding counciller, the TT had caused the nipple trauma, which led to everything else, thankfully all i well now, but i will never forget that burning pain when feeding, and really feel for you, good luck x

My sypmtoms were absolutely identical to expatmoms. I could have written the story myself it was so alike! My baby is 9 weeks old. I've had Diflucan/Flucozonale, grapefruit seed extract, friendtly bacteria tables all the anti thrush things and still no joy. Ive had the same problems with all my children which led me to giving up breast feeding a lot earlier than I had expected.

I would be very interested to know did you get any help or what the outcome was?

Raynauds? Thrush? Bacteria infection?

I had similar excrutuiating pain throughout feed with dd1 and dd2 (I'm afraid I switched to f'feeding, because i couldn't cope). It was partly because of a tongue tie (with dd1), and thrush(me and dd1), but there was also the same blanching of the nipple and also a little blueness. It also hurt when i expressed, and the only way i can describe the pain was that it was like someone running hacksaw blade accross my nipple.

I only heard about raynauds recently when someone mantioned it on Mumsnet, and it struck a chord,especially as I have nipples that are sensitive to the cold. There is a bit written about it on Kellymom, which i think also gives some tips on how to improve pain (like warming nipples before a feed).

I hope you can get some help, because I know how horrible it is to dread each feed. Well done for getting so far.
