Please help! 5 day old baby, cracked bleeding nipples, baby refusing feeds

hi, bit of a crisis situation here, I will speak to my midwife about it as soon as poss today too of course but hoping the wonders of Mumsnet will be able to help.

Had my baby daughter last Wednesday, lovely natural birth, second child. She has been feeding well with lots of wet and dirty nappies but it has been increasingly painful. Yesterday she started refusing to feed and I realised my nipples had started bleeding. Only on one side to start off with, so I fed her from the other side overnight and first thing, and expressed from the bleeding one, but now both sides are bleeding. She was hungry this morning of course, so I fed her some of the expressed milk from a medicine syringe to keep her going. Don't really want to use a bottle at this point as there are obviously already latch problems.

I have already been using lots of Lansinoh, keeping nipples aired as often as possible, rubbing a few drops of milk into them after feeds. I knew there was a problem with her latch because of the pain and have had lines across the middle of the nipple where she has squashed it, so have been working and working on improving latch and positioning but can't seem to improve it. I did have pain with feeding my first child but no bleeding and just gritted my teeth and got through it - not an option this time since she started refusing.

Please any advice as to what to do next would be much appreciated, I don't know how to do this mothering stuff without breastfeeding! and her brother has food allergies and eczema which makes it even more important we get this sorted.

Maybe you could ring one (or more) of the counsellors, phone numbers on the right hand side of this page

Try having a warm bath and feeding her in the bath; this helps the milk to flow. The skin to skin will also help.

Baby should have a large mouthful of breast, not just nipple. Start tummy to mummy, nose to nipple to get a good postion.

SA - if you can bear to trawl through Green Fairy's posts as she had problems she phoned La Leche and things seem to be going OK.

thanks sophie and all, will do. Tried all the helplines last night but just got messaging services, will try again today, thanks for link. will try bath idea too. We've been doing tummy to mummy, nose to nipple, but it doesn't seem to be enough - she just doesn't open her mouth wide enough to be able to get the nipple right to the back.

Can you get someone to ring the helplines this morning for you and hold on until they get through, then hand you over? It can be a long wait at times and you have enough to do already (what with having a 5 day old baby and that!)

Thanks so much for looking those up for me cmotdibbler, that's really really helpful.

The midwife has just been and managed to get her latched on for a pain-free feed, first one in 5 days where I haven't had toe-curling pain, yay! so we are both much happier. She said that dd doesn't actually seem to do that wide-open mouth I've been looking for, she goes straight from wide-open crying to sucking, so while she wouldn't normally recommend latching a crying baby on, that is going to be the most successful strategy atm. She is sending a support worker tomorrow to see how we get on and coming back Wednesday, and is sure the infant feeding advisor will be in touch too. Hopefully downward slope from here on in ...

systems, if you have to feed before you get in touch with a helpline etc, try to get your baby to self attach.

your baby has quite a few instinctive behaviours that aids self attachment and our 'helping' of them can make us end up with latching problems.

relax in the bath. put your baby's head between your breasts, let her find a nipple by simply wriggling and shifting there herself. Your 'help' should be limited to keeping her safe.

you can do the same outside the bath, with pillows half propping you up on the bed. minimal clothing up to for you and baby is advisable, early days till things improve.

Glad to hear you're feeding without pain. I used to latch DS on when he was crying too as once he started crying that was it - he was going to cry until there was actually boob in his mouth.

Glad to hear you feel more confident - i think that is half the battle in the bf!
