Overeating, colic, wind or silent reflux?

My 3 week old formula fed baby appears to be struggling when feeding and nothing I do seems to help him.
He’s farted loads since the day he was born and has never had any problems filling nappies but he seems to be getting worse for gulping milk down and then being uncomfortable. We wind him but sometimes nothing works until a couple of hours later when he’ll suddenly burp.
We use paced feeding but he can still drink 4oz in a super short space of time. He will gulp and grunt while feeding, then scream when the bottle is taken away to allow him time to swallow. He is fed on demand rather than in a routine but this means during the night he can feed every 90 minutes, sometimes he will only want an extra oz but other times he will want more than that. He hiccups a lot but isn’t regularly sick, he has only ever been sick after around 4/5 feeds which suggests he isn’t overeating?
He just seems stressed all the time, clenching his fists and grabbing his face.

It does sound like he's just a guzzler to me if nappies look fine. The night waking is totally normal, as if the variable amount. For burps, how long do you try and get one out for? My little one sometimes would take 10 mins or more of walking and parting but I knew the burp was there. I'd also recommend putting him in a baby carrier after a feed and going for a fast twenty mins walk if he hasn't burped as that was pretty foolproof for my windy babies x

I also found best way to wind my babies who never seemed to burp was to just walk around with them over my shoulder.

I've had this same experience with babies. It does get better after a few months, I think their digestive system just matures and it resolves itself but it's hard going through it.

His nappies are fine, he hasn’t had as many dirty nappies over the past couple of days as usual but he still has about 7/8 across 24 hours - the HV is happy with his output.
We can wind him for 30 minutes sometimes and he just won’t give in then suddenly out of nowhere, he’ll settle down to relax or sleep and let out a huge burp or fart out of nowhere.
I’m getting a carrier tomorrow so we can try that! Thanks.

It’s just horrible seeing him so clearly uncomfortable but demanding more milk - I hate giving it to him because I know it won’t make him feel better but he is genuinely still hungry. He fights being over our shoulder for ages but the relief in his face after a burp is amazing.

Have a look at wonky winding it might help him burp.

Does he have a dummy? It may be that he just needs to suck and soothe himself.

We tried lactose free formula and the difference was obvious within 12 hours

We had this too - our midwife suggested infacol and this has been helping us so far. Ours was really struggling not burping etc but the infacol has helped a bit although he seems to be more of a farter!
For ref our baby is 4 weeks this week and is on 4oz every 3 hours roughly.
