Gas from too small a teat?

my little one is nine weeks old. For the last three weeks, I have been combi feeding, formula and expressing breastmilk and only actually breastfeeding in the middle of the night. I have been using the mam bottles with teat size 0 but in the last three days, my little boy is becoming really gassy. He cries a lot in the middle of feeds, but patting his back doesn’t seem to get much wind out. I stop massage his tummy and bicycle his legs. Sometimes he is ready to go again, other times he turns away and continues to cry. It is making feeds really difficult. This morning 150ml took over an hour.

Can two small of a teat cause gas? I think I should try moving him up to a 1, just worried about making the situation worse.

My midwife said to heat up a sewing needle over the gas burner and make two more holes. Something to do with breastfed babies and nipples having more outlets.
