Laryngomalacia + Projectile vomiting + CMPA ?

Hi - I’m a first time mum so please be kind if anything I ask or say sound silly!

My little girl is 5 months old and weighs 11lb 7oz. She’s had failure to thrive since she was days old. For context, I had a premature labour at 36+4 and she was born 37+0 with jaundice kicking in within hours with doctors said is uncommon as usually within a few days. Anyway she had photo-light therapy and a trip to resus after choking on vomit through our first night in the ward.

Ever since she’s been an extremely sicky baby, and I mean entire feeds. Recently she’s started projectile vomiting entire feeds so hard it comes out of her nose and she can’t breathe. She started off on Cow&Gate but the sickness and constipation was so bad we swapped to Kendamil. We were then referred to our GP who prescribed gaviscon which made her poorly so we ended up in A&E and were later referred to a dietician. To note she also has Carobel as she chokes easily.

Our dietician prescribed Infatrini, a high calorie milk and because of it she’s finally hit doubt digits in weight at 5 months old but was still extremely sick, mucusy poos, choking etc.

We got an urgent referral to a paediatrician after she started having rashes who told us they suspect she has Laryngomalacia and said it would resolve with age. For anyone who doesn’t know, they wheeze awfully with said condition and my baby really sounds like she’s ran a marathon! I mentioned how our GP and HV we’re concerned about CMPA due to her symptoms and showed him images of my daughters stools, photos I’ve got of the insane amount of sick and the rashes and he told me as she’s gaining well on Infatrini he wants to keep her on it and reassess in a few months. He prescribed Omeprazole and sent us on our way.

Flash forward to today, our HV observed a feed where our DD only drank 2oz and choked 4 times on it. Like mentioned, her milk is thicker than standard formula and she requires it even thicker with Carobel but still chokes despite being on Size 2 so teat size isn’t the issue, it’s the floppy larynx. She said she’d write to dietician again.

We came home and she’s been projectile vomiting multiple times. Omeprazole is as useless as Gaviscon for her, she’s still poorly! My partner had to come out of work because she had a bad choking episode once again and to be honest I’m broke with at all tbh.

I just want to know if anyone has been through anything similar? And is it possible for me to request a 2nd opinion from a different paed? I can’t watch my baby be in pain with poos, choke on every feed, vomit forcefully 24/7, not put on weight etc. She’s so distressed I can’t watch her suffer anymore but we’re passed from pillar to post.

Obviously it’s not my daughters fault at all but I’m suffering from PND and under MH team because not only am I adapting to motherhood at 20, I’m watching my baby suffer everyday 😞 We were considering taking her to A&E tonight and putting our foot down for somebody to assess her but me and my partner aren’t like that at all, we don’t like making a fuss but for my baby I will obvs. I feel like they just kinda think I’m a first time paranoid mum but there’s multiple women in our family with kids who are also concerned!

Lots to unpack here but I am not at all suprised you are finding this hugely stressful (my mental health as a new mother would have absolutely tanked if facing these struggles on top of all the other aspects of motherhood to adjust to). You sound like a wonderful advocate for your daughter and are absolutely right to push for help. Priorities for now are to ensure that she isnt dehydrated after what sounds like a day of copious vomiting. Absolutely I would take her to A&E now if this is in any question. This isnt about making a fuss or a reflection of your mental health - any clinician should absolutely take this history seriously. Aside from the dehydration my feeling is she needs to be checked thoroughly to rule out aspiration of feeds and also potential cows milk protein allergy. Has she had her swallow properly assessed?

Thank you so much - I really needed to hear that 💐She hasn’t had her swallow assessed no! Thank you, I will mention that as I stupidly didn’t even think 😆 I definitely agree regarding CMPA but the consultant doesn’t want to try her on a hydrolysed milk as she’s gaining on high calorie so wants to get her weight up before considering that but I can’t just watch her choke every feed in the meantime!

Hang in there, it will all get much easier in time I’m sure. You are in the trenches now but keep reminding yourself that this too shall pass.

Have you had a referral to a Speech and Language Therapist? They will be excellent at assessing her swallow and advising re best way forward for her choking during feeds etc. Personally I would really push for this if she is requiring thickened feeds and still choking and spluttering with those.
