Blocked ducts, blebs, mastitis

Has anyone got any top tips for blocked ducts? They are making breastfeeding an anxious and painful experience as I get one most weeks now which I then have to struggle to clear. I have had mastitis and several lots of antibiotics. Baby is 7 months old and having some solids but is not that consistent with eating and not taking bottles so I need to keep nursing alongside the solids. I am finding that the ups and downs of weaning are leading to big variations in how much nursing my baby wants to do from day to day which doesn’t help. I often get blebs which I can usually clear with difficulty and then the lumps go down but the lumps are huge (varying from plum size up to orange) and stick out through the skin. It is so painful. I currently have an enormous lump like a banana all down the side of one breast. No visible bleb this time despite a lot of poking about in the bath.

One day breastfeeding is going so well and the next I’m battling these huge lumps. I just dread it all the time now - that feeling of a water balloon inside. I go to the GP if I can’t clear it in 48hrs. GP prescribes antibiotics and refers me to hospital for a scan. I wait ten days or so for the scan and the lump is not there when it comes to the scan. This has happened over and over again.

Things I have tried:


  • massage with hand expressing in hot bath

  • blitzing with electric toothbrush

  • taking lecithin

  • working hard to ensure baby has a good latch

  • rubbing with hair brush

  • no bra / very soft large bra with no wires

  • changing nursing positions to put chin against lump and also dangling over baby.

  • baths salts in haaka pump

  • rubbing nipple with flannel to try to clear any blebs

  • sleeping on back

  • not leaving too long between feeding

  • not using a sling or bag over my breasts.

  • reducing saturated fats

  • Drinking loads of water


I would be so grateful for any advice. I just want to get on with enjoying my time with my lovely baby and not spending so much of it worrying about my boobs.

Wow it sounds like you have tried so many things! Did anyone check your baby for tongue tie?

Oh I feel (some of) your pain - don't think I ever had it so bad. Had a long lasting bleb, that I eventually worked out with a sterile needle.

I just wanted to ask if you're using the haakaa a lot? I used silver nipple covers - supposed to help healing. But the way they were shaped - slight concave - meant that they caused me to leak on the side I wasn't feeding, whereas if I took them out while feeding I didn't leak much. I wonder if it caused oversupply and blocked ducts.

It's probably a crap theory, but it's a thought, and if, like me, you're clutching at any straw, no matter how unlikely sounding, might be worth considering?

Thank you so much for these links and resources. Pretty clear that it’s subacute mastitis from the info on the links - I’ve only had these issues since I first got acute bacterial mastitis when my baby was twelve weeks old and then have had them almost ceaselessly since.

My baby did have a tongue tie; we didn’t get help with this until quite late so I think his way of feeding is now a bit ingrained but we have worked hard in getting his latch as good as possible. He is a real breastbeastie and feeds for hours.

The fermented foods info is worth following up. Thank you so much.

And barneymcgroo - thanks for this. I only use the haaka with Epsom salts in to clear blebs, not all the time. I have tried the silver shields but found them very chilly. No problems with leaking at all. Maybe that’s part of the issue? Perhaps I’m more prone to blockages because of this.

Well done banishing your Bleb with the needle, and thank you so much for the encouragement.

Worth looking at these articles.

I would also consider revisiting the tongue tie as a contributing factor and see if an osteopath could help.

Sending you some solidarity, OP, that sounds like really hard going. You're doing wonderfully.

I found that antibiotics for a postpartum infection seemed to throw everything out of whack, and we got into a cycle of thrush, antibiotics, etc.

Fermented foods is a good idea, I think, to try and heal your gut bacteria if you've been having lots of antibiotics.

I also wonder if your baby's latch could do with a bit of improvement - if baby is feeding for hours it might be that they're not draining the breast effectively? Have you a local group/midwife/peer support you could ask to check?

Cranio osteopath is a good call, too, that helped my first with his latch. Find someone with a good reputation and experience!

It may be something that takes a few different approaches in combination to resolve fully. Hope you find things that help!

Thank you so much to to those who took the time to post with ideas. I was feeling pretty defeated with how hard and painful it all was and your messages gave me a boost.

I thought other mums struggling with lumps and blocked ducts and mastitis might like to hear how I am getting on as I had tried all of the usual advice by the time I posted and was getting no better at all.

The link in the first response I received to brand new guidance from the Academy of Breastfeeding medicine (www.bfmed.org/assets/ABM%20Protocol%20%2336.pdf) has proved a real eye opener. It overturns a lot of the usual guidance on mastitis management, in particular the exhortation to empty the breast that I was given time time again by breastfeeding counsellors, GPs and the breast surgeon at the hospital. I had been diligently pumping each night after getting my baby down to get “empty” and thought I was helping to keep the recurrent problems at bay, despite the fact that I was dealing with these problems every week. The new guidance says trying to empty the breast is not helpful and does not advise pumping to achieve this. The idea seems to be that pumping stimulates milk production without emptying the milk as efficiently as baby anyway so creates more of a problem than it solves.

The document also advises against massage or electric toothbrush etc to try and jiggle masses about, or niggly interventions to remove blebs, or feeding lots “through” the mastitis pain and blockages on the affected breast - again, all things I had been doing. The key argument here in the new guidance is that many of the problems stem from inflammation in the breast tissue and that all of these issues make this worse.

The document also looks at lymphatic issues with swelling and inflammation and says that a supportive bra can help. Again - I had been avoiding bras when the lumps were bad on GP’s guidance. I bought a bravado body silk bra which is comfy and have been doing very gentle lymphatic drainage type massage in the shower.

One therapy advised in the new guidance that appears to be evidence-based is ultrasound. I have had this on both breasts now which was pricey but will have been worth it if I can continue without all of the horrible issues I had been having. At present I am three weeks clear in the breast I had the ultrasound treatment on first and two weeks clear in the other one with no issues on either side since beginning this new regime in line with the new guidance and since having the ultrasound therapy. I would usually have had several days of fevers and struggling with lumps in this amount of time. Fingers crossed things are really improving.

I’ve been taking the probiotic advised in the document as well as upping my fermented foods.

I had also ordered a tincture made by la vie called duct flow which purports to sort all issues out asap. May be magic beans type stuff but something I am now doing is working.

Anyhooo. This is enough from me, but I thought worth updating simply to encourage other women to read the guidance in the link
babythrive posted, which differs in all key areas to what seems to be the norm advised here. Good luck to any others with similar issues. Fingers crossed I’m on the mend now and can continue to breastfeed my baby cheerfully and healthily.

Back in the 90s they used to reccommend a cabbage leaf in your bra next to your breast to help with pastis, even the midwives.

Hope it continues to improve and you can breastfeed for as long as you want to.

Hello. I have found something that helps more than all the stuff above - absolutely rigidly starting on the opposite side to the one I began with the feed before. I offer both sides at each feed. One of my breasts always feels fuller and more in need of feeding from and seems to produce more milk so I often started on that side for the relief of feeling it empty. Most of the blebs were on the other side - always in the same duct it seems, or always the same pore with the Bleb at least. Now I have started beginning with this side just as often, the blebs are more like once every several weeks rather than all the time. Good luck!
