Baby twisting head and using boob as pillow whilst feeding

My 4 month old baby has decided that my right boob is to be used as a pillow whilst feeding, and will twist his head round to rest on the centre of my chest, taking my nipple with him so it's pulled sideways. He's been finding his own latch for a while now, but this position makes it a terrible latch, sore nipple and the pressure of his head is giving me blocked ducts. I've tried different positions holding him, but he just twists his head round. I've also tried pushing his head back round straight in line with his body but he just resists me and goes back when I stop. He seems to drain my boob well when he wants, but swallows more air. He doesn't do it on the left breast. Any suggestions?

Can you slide him over so he’s under your right arm and in the same position as he would be on your left breast?

I could try, he's a bit big maybe for that now but worth a try! He's started complaining about being sideways feeding unless he's sleepy (on both sides) which is maybe why he's twisting his head, and prefers to be upright down my body at the minute. Slightly easier on the nipple when he twists but being upright makes it easier for him to turn his head.

IME they just do gymnastics like this the older they get Confused It's not always comfortable but if it's not awful I just leave them to it.

Have you tried the rugby hold?
So if you're feeding from your left breast you have him on your left side, not your lap.
Tuck him under your arm like a rugby ball, or if he won't lie down, plonk him next to you on the settee in a sitting position, on a cushion if needs be, and wrap your left arm around him securely.

Oh no wait, sorry, missed about blocked ducts - yes I would try the rugby hold on the other side.

At least the rugby hold will pull my nipple in the opposite direction 😂. I haven't used it since he was a newborn! I couldn't get comfortable sitting with him like it as there wasn't enough space to the side/behind me for me to feed without sitting up the entire time. I shall see what happens now he's huge 😂
