Slow to gain breastfed 7 week old - possible diarrhoea - advice please!

Hello ladies. I'm a long time lurker who has benefitted hugely from reading posts and I'm now posting for some of your valuable advice.

My darling tiny girl is 7 weeks today, (but was born 3 weeks early) and she's pretty slow at gaining weight, a fact which I'm pretty sure may be compounded by what I think may be diarrhoea - although difficult to tell in a bf baby. The scales which gave her weight at birth were almost certainly wrong - she was a small baby and we were told 7lb 11, but 48 hours later she was weighed and was 6lb 10 - an unfeasibly large weight loss in a very short time for a baby which fed well from the start. I think she was more likely to be just under 7lb at birth. From day 3 she has gained, not lost, albeit slowly. At 5 weeks her personality and nappies changed. My smiley happy baby became fussy and screamy, demanding constant feeding and crying as soon as she came off the breast. She also wasn't sleeping enough in my opinion. At the same time her poo began to smell different, looked a bit darker, more brown rather than yellow, contained mucus, and she began passing only 1 or 2 poos a day instead of several each day. At the time we put it down to a growth spurt and after 6 days of this she seemed to have slowed down the feedings a little, was sleeping better and napping in the day and was more content, although nappies still the same. however she has had another "relapse" over the last couple of days, colicky screaming, soothed only by feeding (so we do that ALL the time) but more screaming within minutes of a feed ending. She really seems to be in pain and the screams break my heart. She gained only 3.5 oz in her very fussy week, and only 1.5 oz last week. My HV is having conniptions over the blooming centiles but my GP put it down to colic and seemed unconcerned about both the weight gain or the runny poo. however, I have just poured the snotty (sorry) contents of her nappy into the specimen jar which I'll have sent to the lab tomorrow and I'm sure she has a tummy bug which just won't shift. Could this be causing her slow weight gain? she is only 7 lb 13 today.

Her feedings, though frequent (every 1 to 2 hours in the day, every 2 to 3 through the night) seem fine, her latch is good, it has been observed by midwives, HVs, paediatricians, lactation specialists and bf counsellors amd pronounced fine! I've had no breast or nipple problems save for a couple of blocked ducts caused by a dodgy bra. aargh! to top it all off, dh has discovered he has shingles this week which has had me worried about her catching chicken pox despite my antibodies from the bf.

please help!!!

oh my dear how awful .
it does sound like she might have some sort of tummy bug. I think you are doing all the right things - feeding on demand etc. it sound slike your latch is good. a slow weight gain can be normal for you baby.
are you due to see the hv or gp again? I can't really think what else to suggest apart from (a) keep feeding and (b) get the tests you are planning and (c) look after yourself - get lots of rest and keep eating well.
how very worrying for you, but remember your baby is gaining weight, she is feeding well and you are getting medical advice.
if she does have chickenpox, then hopefully it will be a very mild case.

I agree with harpsi, carrot - it seems clear your baby has a tummy bug, and it's worth thinking about the fact that she'd be a lot poorlier if she was not breastfed. Throughout this difficult time, she has continued to gain weight (I don't know why your HV is worrying - the weight gain is within normal limits as far as I can tell from the info here). Babies who are fighting infection do tend to gain weight more slowly, so her 'real' weight gain might have been somewhat greater, it's true.

She is unsettled and screamy at times and appears to need a lot of tender loving care from you, which ties in with the notion she is affected by a gastric infection...and you are doing the right thing in getting this checked out.

I hope it gets sorted.

Thanks Ladies for your responses. It's so disheartening to be feeding so frequently, and to not see any obvious problems with feeding, yet for her not to be gaining much. I co-sleep, do skin to skin, carry her in a sling, feed on demand, no dummies, no bottles. I've tried to express using a manual pump to further stimulate an increase in supply,but I can barely produce an ounce and I think I may need to try a different pump as this one seems to rub the end of my nipple and on the occasional attempts I've used it I have developed blocked ducts afterwards - I realised this today when I got another one last night. I'm considering the galactogogue route next!

DD seems a little happier today, I'm getting my smiles again, so I'm hoping things are on the mend, but don't want to speak too soon.

As for seeing the HV / GP again, I'll follow up on the lab results when they arrive and see what to do next. I might wait 2 weeks before another HV weighing as she's making me anxious over every little fraction of an ounce when other than this suspected bug, dd is gorgeous, tiny but mighty!

new mum paranoia...I veer frequently between temporary confidence that I'm making enough milk and anxiety that my tiny bub is on teeny rations because I'm not making enough (have always had tiny boobs & they have an inferiority complex). I also would like at some point in the future to be able to have dh or grandma give a very occasional bottle of ebm to allow me a little time off for good behaviour eg. 1st wedding anniversary at end of October.

today has been much better as a matter of fact, happy baby, long walk into town with dh and bub in sling, meal in restaurant and bf in public - get me!!! dd took longer naps and had more spaced out feeds; 2 to 2.5 hours-ish...great progress for us!

Sounds like you´re doing a fantastic job, you should be really proud of yourself!

Re not making enough milk question, size of boobs is irrelevant. Big boobed women may only have small milk capacity and small boobed women a larger capacity. Hence some need to feed more often to replenish milk supplies, others have an ample supply and don´t need to feed so often... Good luck to you! Hope baby is better soon!

LO is due her 8 week vaccinations on Monday, but the lab results aren't back yet. If they're not in by tomorrow's afternoon appointment, I'm wondering whether I should delay the jabs in case she is struggling to deal with an infection. She's really variable, some days she seems OK, others she really seems to be in pain. Last night she had a rough time, was inconsolable - even when offered the breast - a first for us. Today - OK again. Maybe it is simply common or garden colic. Wish I knew!

As a first time mum i'm no expert but my ds was born weighing 6lbs 12oz - 10 days later we were referred to hospital by our mw as he had gone down to 6lbs 3oz (over cautious as it was still within the 10% range) He fed normally but seemed to lose alot and was very slow to put it on. Only an ounce or 2 a week and it took 6 weeks to get him back to birth weight and into newborn clothes! and he stayed on the bottom line of the percentile charts for ages. HV's etc were very fussy about this but I knew he was gaining weight and therefore healthy and I was doing well with the bfing. It does sound like he might have a wee bug from the nappies but a culture will determine that. Ds had a little one - but I see it as a good thing as I'm sure it builds up their resistance!
As far as the constant feeding goes, I was told to expect growth spurts at 2,4,8,12 & 16 weeks - my ds was at least a week early for each one - with the 12 wk being a particularly bad one. Anyway - just wanted to say that you're doing a grand job and despite the bugs they get, l.o's are really resilliant. My Ds is now 5 months and in between the 25th-50th centile - quite the fat little knacker lol

Just read your last post - I was always told unwell meant a cold/virus etc rather than tummy bug etc but you could ring in the morning to make sure.
Also, re the inconsolable bit - have you eaten garlic or onions recently??? (or anything you dont usually?) I only ask as I have had the same prob with ds occassionally in the past month and it coincides with Gorgeous yummy garlic bread consumption! Another sign for me was excess possetting the following day.....Wont be eating it again now until I stop bf
