Breast pumping- supply issues

I have really flat/inverted nipples. DD1 never managed a latch despite loads of help with positioning etc. The midwives always started by saying ‘it’s not nipple feeding’ but no one was able to get her to latch. I pumped for the first while but wasn’t sustainable (for me)

Fast forward to DS1 - currently 8 days old. Exact same problem with him.
he was born on Saturday. No latch establishes in hospital. He was getting very frustrated so they suggested topping up short term.

Midwife on Tuesday suggested nipple shields. They didn’t work with DD1 but he’s been great with them in that it’s really helped him suck but he’s still coming off ravenous. Midwife doesn’t know if it’s because he’s not actually swallowing (she said she can’t see a swallow) or is it because my milk behind - on Friday it was still Colustrom however think transitioning now.

She suggested pumping. I was really reluctant to go down this route again (really got me down the last time being stuck to pump every 2 hours and having to put DD1 down). But because his wee suck seemed good I thought I’d give it a go.

Bought a pump Friday afternoon (so day 6). Bought fenugreek too - been taking 9 capsules a day of 600mg

Been pumping 2-3 hourly since. Volumes still rubbish. It’s a double pump. I’m getting 30mls per session.

We’re now on Sunday and no improvement. Desperate to try and keep it up before my milk completely swindles. Letting DS1 ‘feed’ in between and attempt him on boob before all his feeds.
Tried a hot cloth and massage etc.

Is there anything else you can think of? Anything I can do? Anything you’d recommend in terms of how long I pump for etc. I’m hoping it’s not too late but starting to feel like I’ve akready left it too late.

Any advice would be hugely appreciated.

I'm 13 days post-birth so I don't have long-term answers but one thing that's made a huge difference to my pumping is having the baby skin-to-skin while I'm pumping. Do you do that?

I also found the first 3-4 days after my milk came in (it came on day 3) only tiny droplets were being extracted by the pump and I had no idea that wasn't normal, whereas now, 10 days (ish) later I'm getting little streams of milk when I pump, so I think it must have taken a week or so for letdown to happen at a reasonable speed.

Honestly this time 3 days ago I was ready to quit because of my own problems and I'm glad I kept trying. Good luck and I hope it all works out for you.

30ml is really good! Especially so soon post partum. I started pumping when DS was 2weeks and that’s all I got each session. Now I can get double that at least but it takes time to establish supply. Also I’ve noticed that if I’m tense, almost nothing comes out, but, deep breath and relax and letdown happens much easier. (This is just what I found, not saying it will work for everyone!) I’ve been taking fenugreek for past week, I think it makes a difference, not huge, but definitely more milk each time. Good luck! 😃

This is going to sound weird but it can work. Cover the bottles in socks whilst pumping. Don't look at what's coming out. I know several people this worked for, it seems to be because when we can see the output we stress about it.

Thanks so much for these tips - really appreciate it.

@johnlapsley will try that sock trick

@MilkLady02 - didn’t realise 30ml was average - that’s really given me a boost hearing that.

@WhatTiggersDoBest - that’s really good to hear your story and has definitely spurred me on to keep at it. Need some patience I guess but it’s not something I possess a lot of! Thanks so much for taking the time to reply - really appreciate it.

Could have wrote your post myself! Dts in nicu so had to express. Supply was shit and stayed shit. Never more than 40ml any session, total volume, not from each side! Mostly from the right boob. Left boob stopped cooperating fairly soon after starting.
I would then get so stressed on seeing the other mums expressed milk in fridge in unit.
Thought it might have got better once home with the boys but no. Gave up after a week as couldnt take tge stress anymore.
Hoping you have a better time than me. Thats great idea re sock from pp.

@hairwizard - it’s not easy is it. I got so stressed with DD1 about it and swore I wouldn’t this time round. But here we are!! DH off on pat leave for another week so think
Il keek going for this then if still
No joy call it a day. I’m a firm believer in happy mum happy baby and being tied to a pump every 2-3 hours is not making this mum s happy one! Thanks for your story, it’s always good to hear of people in the same boat.
