Screaming at the breast

Hi all.
Very very sleep deprived and sad mummy here. My 9 week old has been a nightmare to feed from the start. Posterior tongue tie divided twice, suspected thrush which has been treated and silent reflux. She swallow and gags and gurgles all the time though doesn’t seem overly distressed unless it affects her sleep (which it does, so she is)
And all of that I’ve got on with but over the last few days she is starting to scream at the breast and refuse feeds.
My gut tells me it’s when she is tired and trying to sleep but my fast letdown is upsetting her, or it could just be painful she arches her back a lot, or I guess something else. She often ends up falling asleep after a long bouncy cuddle after sessions like this and I don’t know if that’s from tiredness or exhaustion because she’s so unhappy and has been crying. It’s often in the evenings but recently has been in the morning too.
I’m so worried about her!
Has anyone any tips? We have an appointment pending with a paediatrician under our health insurance but it isn’t for a while and I’m at a loss.

My wee one was like this. Turned out to be a cows milk allergy, I cut dairy out of my diet and she was a changed child in days. Also, have you tried nipple shields, might be easier for them to latch on.

Gosh really? I’ve debated whether that could be the case. Some of her symptoms fit it’s just so hard to know if it’s fast letdown or reflux or allergy or all three!
I’ll cut out dairy and give it a while and see if her symptoms ease at all

Yes do try cutting out dairy to see if that helps. If that's the problem you'll see a dramatic improvement

Other thought - has gp ruled out floppy larynx? (Laryngomalacia) Is there any marked sound at night like a donkey noise? Floppy larynx can cause lots of swallowing issues and reflux, which is so painful for them and needs medicine to help (ranitidine or omeprazole from memory)

Assume yve tried baby gaviscon before a bf and infacol?
