Moving on from feeding to sleep at 12/13 months

I'm looking for advice on gently moving on from breastfeeding to sleep. My DD is 12 months old. Around 7 months I started rousing her after her last feed before doing PU/PD and it was goign well but then she got terrible eczema and it took months to get under control. During this time the only way to soothe her and stop her scratching was to allow her to BF to sleep in my arms and then put her down which I did. But obviously it put us back to square one.

She still wakes at night but usually only once for a quick feed then goes back down without a fuss. But bedtimes are hit and miss and she can take a while to nod off. I really want to start moving towards her self settling after a feed at bedtime - and so that daddy can do bedtime with a sippy cup of milk too if I need to go out.

How should I start moving on towards from bf totally to sleep? I want to continue bf on demand (on the days I don't work) until at least 18 months, and hopefully 24 as per WHO recommendations if I can. I'd just like to separate bed from boob without hours and hours of relentless screaming if I can (she can be very determined).

It will probably have to be gradual- start with just popping baby off as they've been flutter sucking but are asleep, shush as necessary then gradually less flutter sucking.

My DS is now 14 mo and generally doesn't feed to sleep unless exhausted so after he's had both boobs and is starting to flutter suck/ not actually swallowing, I take him off and we have cuddles then into cot and hold hand/ stroke head/ sing until asleep.

Some nights he feeds to sleep, rapidly as he's so knackered, like tonight
