Refusal to latch

My 6 day old baby breastfed ok for the first 5 days (his latch was a bit shallow and he slept on the boob a lot and had to be constantly chivvied to keep sucking) but as of yesterday morning he has flatly refused to latch on. My nipples are even when erect very big for his mouth and my breasts are quite large and getting him on is a struggle. He now cries and screams if I attempt to get him to latch and will sit with the nipple at or just in his mouth but not latch. As he’s slightly jaundiced and also it’s so hot just persevering isn’t an option as he already got himself quite dehydrated by yesterday afternoon and the Midwife saw him. I have no problem pumping as I have a lot of milk and feeding with a bottle, and have added bit of formula but how am I going to get him back to the breast if he needs to be fed so regularly. I can’t leave him hungry and screaming refusing to latch and if it goes on it gets dangerous. So I’m “taking the edge off” with a bit of expressed milk in a bottle and then trying him on both breasts. This means he doesn’t scream at them like when he’s hungry but he still just sits with the nipple next to his mouth or squirms and spits it out. I want to keep him trying but he’s going to have no idea how to latch if he keeps feeding from the bottle. Would breast shields help or hinder here? I feel if he were a bit older and stronger he’s take the nipple but it’s just too big/too much hard work for him at this moment. I have tried expressing first to soften the breast, trickling some in his mouth from the nipple first, skin to skin all last night etc, still won’t latch today.

Im no expert as I've only been doing it for 3 weeks, but I'd say what've you got to loose? If nipple shields allow you to carry on feeding your baby... It's worth a shot 🙂
Bfeeding is so hard already without struggling to latch your baby. Sometimes mine just wants to have my nipple near him, it's a comfort thing I think.

Have you tried different positions eg rugby ball hold?

I'm big breasted too. I'm not an expert but it is possible to go back to breast after using shields.
DS was bottle fed initially as I was very ill. Shields saved my sanity and allowed me to start breastfeeding and then suddenly a few weeks ago he decided he didn't need them any more.

I’ve tried various holds, is all a bit awkward but the main problem now is his refusal to latch at all even when the nipple does get in his mouth, he just screams and thrashes his head. I was thinking if the nipple shield could get him using the breast and being used to position etc it would be better for getting him back on the actual nipple then just using the bottle and attempting to make him latch for 10 mins while he screams.

I know perhaps I should just persevere and avoid bottles but when he’s so distressed and needs to feed I can’t bring myself to just hold out for breast only and endanger his health. He’s so small.

A small success... I was pumping for him and as he started fussing for a feed I stopped, realised the pump had made my nipple into a tear like shape so I popped him on it and he latched! Still didn’t have what I’d call a satisfactory feed as it was about 5 mins of not constant sucking and now he’s asleep on my boob, but I’m so glad he’s actually shown he remembers how to latch. Will carry the pump with me and make my nipples that shape pre feeds and see how that helps.

Awe bless him, glad you're making progress. Does he have a tongue tie?

No he was checked in the hospital at birth and didn’t. The breast feeding support lady can check today as well. He’s fed a bit overnight and this morning but he’s not staying on anything like long enough to get a full feed out so I’m still topping up.
