Baby screaming at the breast

I’m beginning to find breastfeeding really traumatic and was wondering if anyone could help. My 7 week old baby girl has been diagnosed with reflux and is is currently on ranitidine and infant gaviscon. We think this is under control now as she is generally a much happier baby. However, I do struggle to work out a pain cry from other cries...

For most of the day, she is a happy and contented baby. She sleeps well, naps well, has lots of smiles and interaction, etc. However, feed times are another matter. Because she is on infant gaviscon, I am expressing 1-2 ounces for each feed and giving her that with the Gaviscon mixed in, then giving her more milk at the breast, if she wants it. She takes the expressed milk in the bottle fairly well (sometimes grizzly but generally ok). However, when I then put her to the breast, she is initially desperate for the milk and latches quickly, but then pulls away and literally screams. I struggle to think it’s pain because she has only had about one ounce by that point, as she starts screaming pretty soon after latching. By screaming, I mean that’s it piercing and there are real tears. It’s very distressing and if I try to offer the breast again, she sometimes latches on desperately quickly and the same thing happens again or she shakes her head around and waves her arms around. At that point, I obviously take her off as I think that means she doesn’t want any more but she doesn’t stop screaming. She appears hungry as she balls up her fists as some babies do when they are hungry.

Has anyone experienced this? I’m seeing the Health Visitor on Tuesday and will ask her, but I’m so upset tonight and wonder d if anyone had any advice. Thanks!

OP I am having a similar reaction from my DD and am worrying now it might be oral thrush. Has she got a sort of white scurf on her tongue?

Agree check for oral thrush but is it possible to offer breast first then the expressed mix? Suckling at the breast is harder for them than having a bottle so she may be mad that it’s not as easy, especially if she’s hungry. My suggestion would be to maybe squeeze some milk out first, let her feed for a bit then bottle. It’s a tricky one, definitely don’t wait until she’s ravenous for her feeds, good luck.

That sounds challenging for you both. I hope you’re ok. I would try offering the breast first so that baby has to do the hard work first. Perhaps baby is finding it frustrating to have to wait for milk to come down after starting with an expressed bottle feed. Have you tried different positions? I found that the rugby ball position worked well for my second baby when she was frustrated and hungry. Also, could it be the heat? Do you think trying a feed with baby in just a vest and a fan in the room could help at all? As with everything breastfeeding related... it may just be a phase for a few days. She could be fussing as others have mentioned because the flow of milk at the breast is slower and she has to actively work for it until the bottle. I'd seek skilled face to face support (do you have an infant feeding coordinator at your NHS Trust? If not la leche league or and IBCLC. Midwives and HVs can have as little as an afternoons bf training and GPS less than that!)

You can also use paced bottle feeding so it mimics bf better

Thank you so much for all the advice. I don’t think she has thrush, as I can’t see any white spots or patches on her tongue; I’ll keep an eye out for it, though. I think putting her to the breast before the bottle is a really good idea- I’ll definitely try that. I’ll also contact LaLeche and try to get some professional advice.

Oh yes and also google paced bottle feeding. It is a method of bottle feeding thst is closer in experience to bfing ' makes baby work harder so they don't see bottle as easier and prefer it
