DS is 7 weeks, posterior tongue tie was snipped at 6 weeks. Before this he'd struggled to latch and it was incredibly painful so we'd resorted to using nipple shields. After his division last week we tried to ditch the shields. Unfortunately his latch was still awful and I ended up with mastitis.

I've managed to clear the lump and I'm still on antibiotics. Although my boobs still hurt. The one I had mastitis in is generally achy. Still feels bruised. And I'm getting electric shock type pains in both sides in between feeds. They are just very uncomfortable.

My concern is my supply has dropped. Before this, my boobs would feel full between feeds and I could tell when he'd emptied them. The shield would be full of milk and he'd look satisfied after feeds. At night I would leak a little because it's the only time he'd go 4 hours between feeds. He was having one bottle of formula top up (he was started on top ups in hospital after every feed and we've reduced this to once a day).

Since the mastitis it has changed. The only feeds I can hear him gulping and he looks satisfied are the feeds where he has had a good number of hours in between. Only very rarely will there be milk in the shields. He has been feeding non stop this evening from about 4pm and every time he takes himself off the boob, his hands are straight in his mouth for a suck. He will be inconsolable until I put him on the other boob.

How likely is it that my supply has dropped? Can I increase it? I know the more he is on the boob the better and ideally I need to lose the shields but his suck is so ferocious that my nipples are raw. His fists are permanently clenched and he doesn't relax. I'm worried because I don't think he is weeing as often as he did last week. Any advice? I'm due to see the HV on Thurs.

Hi! First of all well done for feeding through mastitis. Been there and it's awful.
Secondly, I also fed with nipple shields, and didn't mange to get rid of them until Dd was 4 months old. We carried on breastfeeding for a few years :)
It's great baby is feeding lots, if you can bare it, because it will bring your supply up. Eating oats apparently helps too. If you can drop the bottle of formula that will help too, or express while baby is having formula as otherwise your body will make less milk as he is effectively missing a feed.
Breast milk is supply and demand, the more he demands, the more you will supply the next day.
Keep going!
Also, pain in between feeds can sometimes mean thrush, especially if you've just had antibiotics. Keep an eye out for white patches, and if in doubt get treated for it so it doesn't take hold.

First of all I would go treat yourself to a tube of lansinoh, that'll help with your nipples. They will get better as long as he gets his latch right, hopefully this should be much easier for you now though.

your supply will increase, don't worry. Keep him on the boob as much as you can, drink plenty of water and if you do need to top him up with formula make sure you pump for that feed.

You could also try making some lactation cookies, I've found them really helpful for giving me a good boost.

Selly, it'll help to talk this through with someone in real life - try one of the bf helplines.

There's a lot going on. The pain in your breasts is not normal. It could be related to your baby's positioning and attachment, some left-over sensation from the mastitis, even thrush.

There might be a short-lived drop in supply - hard to tell from what you say, because it is normal for breasts to cease being full between feeds once the first weeks have gone by. It is also normal for babies to have periods where they will 'cluster feed' especially in the evenings, just as you describe.

You sound as if you are dealing with this just fine - feeding responsively so he can be on the breast on and off the way you say he wants to do, is the best way to get over this temp. difficulty.

Yes, good idea to check all this out with HV.

When looking for white spots from thrush, is it just the nipple itself or can they be on the areola?

We've fed a bit better today. Boobs are still sore but he seemed to be getting milk like he did before for at least some of the feeds.

There is definitely a lot going on. I've also had an infection because my stitches opened up last week. I've since had on off bleeding for the last week with stomach cramps. I saw my haemophilia consultant today who has asked my GP to refer me back to obs. I'm going to get an appointment tomorrow morning!

try to find your local La Leche League group, the leaders are fab with practical advice. Ours has a facebook page with their contact details, so you can phone anytime for advice. Good luck, and remember you are fab to have gone tgrough all this and still be going!
