We are having a feeding meltdown!

DS is almost 7 weeks. We still haven't managed to fully establish breastfeeding. He struggled to latch from day one which caused me huge amounts of pain and made him very frustrated. At about 3 weeks he would only latch with a nipple shield and this made it bearable for me.

Turns out he had a posterior tongue tie and this was snipped on Monday. We've been trying to wean off the shields since then but it's like we are back in week one.

Today is the first day he has latched with minimal pain. Yesterday it was excruciating. Mostly used shields. Now its still sore and my nipple is still misshapen but I've been working on trying to encourage him to bring his tongue forwards. He is gulping more often and isn't clicking but still does a lot of sucking.

Tonight it's been a disaster. He latches, feeds for a few minutes, throws his head back whilst clamped on my nipple and then howls and howls. I keep swapping sides, re latching but the only thing that works is putting a shield back on. He calms down instantly and I can hear him gulping.

Is there any way of losing the shields now? My supply isn't brilliant so I was hoping we could start increasing it with a view to stopping the formula top ups once BF was established. I tried expressing over the last few weeks but he's permanently attached to me and we were only getting 20 to 30 mins between feeds. Last week he dropped from the 25th to the 9th centile so I know if this doesn't improve we will be advised to keep increasing the formula.

Any advice would be welcomed.. We want a happy and well fed household!

It's a tricky situation to wean off shields especially when you are using bottles to top up.
Would you HV be any good with breastfeeding advise. Can you get some help from a breastfeeding counsellor?

Don't worry anyway - you will sort it out either way. I know people who have moved from using shields at 2 months so it's definitely possible. I also know someone who fed till 8 months using shields so don't panic if you can't drop them. Sorry I can't give you more practical advise. I saw the HV yesterday and I went to a BF drop in today. It's just so hard when it's clear DS isn't happy about the change.

And to top it off, I've now developed a massive lump with red streaks on my right boob. I'd been so careful about this happening when using the shields.... currently attached to the pump to try and rectify this Sad

Were they any help at the drop in? What was the HV like? Make sure you are using the right size shields - the wrong size can cause blocked ducts but it is probably because your breasts haven't been fully emptied recently. Using warm compresses can help unblock the duct and don't forget to take painkillers if your in pain. Massaging towards the nipple with a hairbrush in the shower helped me when I had blocked ducts. Pump after you breastfeed to make sure that the breast is fully emptied to help your duct. Pumping before may cause more problems in the long run.

They gave me advice on latching and building up my supply but no advice on weaning off the shields.

I will check out that link.. I've been using shields exclusively for 4 weeks now and had avoided any blocked ducts. If I felt I hadn't emptied properly after a feed, I expressed. Suddenly I try without them and this happens.. I've taken paracetamol. I'm already on antibiotics because my stitches opened up this week and became infected.

It's just not my week!

Just used a hot compress and I'm now feeding. Going to get DH to run a hot bath.

If I keep feeding with a shield, will that be enough to unblock it? I just don't think I can get him feed well enough without. Plus in about an hour he will go to sleep and won't wake until 6 am.

Hi, sorry to hear you are having problems after the TR division. D'S had his cut last week, and still no improvement. They do say it can take 2-3 weeks. Could you try using the shields for the first part if the feed, then taking it off and offering again? They also help to draw out the nipple a bit making it easier to latch. I feel for you because even though DS will latch, we always have a couple of feeds per day where he is crying and pulling off the breast.

I'm definitely not an expert but in my experience feeding with a shield shouldn't reduce milk transfer as long as it's the right size . His latch just may not be good enough without the shields to transfer milk effectively which is why you got your blocked duct. I'm in a different country so not sure about breastfeeding support available but is there someone else you can go see with specific advise for your situation?

Oh poor you with the infection too. I hope your DH is giving you lots of TLC.
