CMPA, Reflux or something else?! please help :(

This is going to be a bit of a long one! My LO is now 8 weeks, from birth was quite a sicky baby but never really thought much of it, until around 3 weeks when it became quite projectile! At that point she was exclusively breastfed, took her to the GP and he suggested infant gaviscon for reflux, initially there was some improvement with sickness but made her struggle to poop! Went back to GP after HV advised there may be an alternative and he refused to give it without reason why! Carried on with gaviscon as GP advised and giving cooled boiled water to hopefully help with the toilet issue! Had to start topping up with Aptamil Anti-Reflux formula because I couldn't express enough breast milk for all her feeds and GP had said to mix Gaviscon with full feed! She's continued with increasing sick and worse constipation! Now when she goes it's solid! HV got me in to see paediatrician at the hospital and they prescribed Nutrimagen formula for Milk Protein Intolerance, been on this for two days, she doesn't like taking it, it still makes her sick and she passed a small hard poop earlier which had some blood tinged mucous in it?!? Have no idea what to do now? Any help, advice or experiences would be appreciated!

Firstly, I really sympathise with you. We had a very refluxy baby. You don't give gaviscon and antireflux formula at the same time, as they both act in the same way, to thicken the feed so it stays down. The usual second-line treatment for reflux is ranitidine.

It is possible that the blood tinged mucous is due to the solid poo having caused a fissure, but you should get that checked out...please ring NHS Direct. We ended up in hospital due to mucousy poo, but this was a fissure caused by the gaviscon as it can cause constipation.

I would ask for a second opinion if I were you as your doctor diesn't sound terribly helpful.

We were given Nutramigen as they thought Our LO had CMPI, but he didn't...however we found that adding a pinch of banana nesquik sweetened it enough for him to take it...a tip given to us by the paediatric dietician.

I would definitely call NHS Direct or your OOH about the poo, and hopefully they can help you. X

Also, it takes two weeks for the milk proteins to leave their systems, so you might not see an improvement for a while. Are you still BF? Did they advise anything on you giving up dairy if you are? If so, it needs to be done under supervision as you may need calcium supplements. X

She only had gaviscon with breast milk bottles not the formula bottles..
The sickness with these bottles just makes me want to put her back on the anti reflux but I'm guessing it's just because she's not used to it!
I keep panicking about the poo but my partner is so laid back generally he's horizontal so he wants to wait to check the next one Hmm
Feels like there is no near end in sight for my poor little baba! X

I'm still expressing to keep my supply going incase they're wrong and I can go back to it, but not actually feeding her any! They mentioned it, but her attitude was very much how difficult it would be, not very helpful, but when I mentioned the idea to my mum she laughed at me because of how fussy I am with food already! Not sure how practical it would be for me really x

Hi Keely..my 16wo is on Nutramigen for reflux & ?? CMPI . He only gets Nutramigen occasionally though as I BF. (Was 5 wks early so was used to top ups in hrs beginning). I was advised to out vanilla essence in it to make it sweeter & it worked. I stopped all Gaviscon & Losec as they didbt stop his reflux ( though made the sick thicker) & because the reflux bothers me more than him ! The Nutramigen is thinner & my DS always brings MORE back after it. HTH & good luck.

Hi keely my Ds2 had the laughably entitled "silent" reflux (wasn't sick, just contant screaming as stomach acid burnt his oesophagus) which turned out to be linked to MSPI (milk and soya protein intolerance). I did continue to BF him which meant I had to be dairy/soya free. It's hard work initially but doable in the long run. If it's just a cow's milk protein issue and you can have soya that's relatively easy as there are lots of soya replacement products available. He had omeprazole, after trying gaviscon and ranitidine, neither of which worked. The Nutramigen milk is very thin in texture which can initially increase vomiting. it can be used with a thickener called carobel so if it doesn't improve that's worth asking about.
I highly recommend a website called LittleRefluxers which has a support board, and a book called Colic Solved by gastric paediatrician Dr Bryan Vartebedian, which despite the stupid title is the bible when it comes to reflux and CMPI.

Hi, the sick has definitely gotten worse since we started the new milk but I do think this is because of how thin it is, but reluctant to add the gaviscon in cause that causes the constipation again as had a few really bad constipation poos and then one normal one this afternoon,wahoo! Will deffo check those two things out, really need to know more! Thanks :) x
