Silent reflux baby feeding problems

Not sure if there's much I can do about this, but wanted to ask for advice.

My ds is 20 wks and was diagnosed with silent reflux at about 4wks. At 6 wks or so he changed from comfort feeding to feeding refusal and we had a really hard time keeping his weight up. He's currently on Gaviscon and 15ml omeprazole (ranitidine didn't help) which had been controlling things to a manageable level. He's on about 40th percentile for weight and been steady for a few weeks.

However the reflux has got worse again so for the past few days it's been very difficult to feed him. He will work himself up into distress when hungry, take a few minutes on the breast (or 30-60ml from a bottle) and then refuse to drink any more. Gradually he gets distressed (often to howling point) because hungry and uncomfortable, but can often still be distracted or comforted. He's crying in his sleep and waking more at night which I assume is down to hunger. We've been using dream feeds both day and night to get normal-ish sized feeds into him, but then he gets trapped wind and is incredibly difficult to burp when asleep. And for the past 24 hrs he's getting harder to dream feed. He won't sleep, and when he does, wakes when the feed starts.

I really hope this is a short-lived blip but whilst it does, does anyone have any feeding tips? So far he's still alert and having wet nappies but I do worry about his milk intake. I've been to the gp who say it must be reflux as they can't see anything with him, and have a paed appointment next month.

Sorry, should've mentioned - he's prominently breast fed but takes a formula feed in the evenings, plus top ups at other times if very hungry.

Poor you. The only things I can suggest are feeds little and often (tiring though), keeping upright after feeds (I'm sure you do this already) and feeding when sleepy - just woken from a nap might be better than dream feed for the wind.

Do you notice him being worse after the formula? I'm wondering about dairy allergy.

The other possibility is that it's teething that's making him refuse feeds (and waking more at night) and not to do with reflux at all. Any signs he is teething?
