When is it wind and when is it silent reflux?

DS is almost 3 weeks old and is EBF. About 3 days ago he started getting really upset after feeds and at random times in the day and night. He screams and cries in pain, arches his back, stiffens his legs and makes a horrible straining sound. He doesn't do many burps but lots of big farts and poos. He is putting on lots of weight and doesn't vomit. For the first two weeks he would feed every 3-4 hours but now this is every 2 hours in the day and more frequently at night...I'm not sure if this is more for comfort rather than genuine hunger.

Sometimes holding him upright after feeds helps, sometimes it doesn't. I have propped his mattress up and try to wind him after feeds.

How do I know if this is normal wind or if it is reflux and needs advice from a doctor? Winding him doesn't really produce anything. He will fart after lots of crying and writhing. Should I try infacol? A dummy?

The constant waking, feeding and screaming at night is already getting me down (also have a toddler to look after). Any advice please? Thank you.

When DD2 was about 3 weeks I was convinced she had silent reflux, she was in obvious pain and kept screwing her face up as though she had a bad taste in her mouth. I did the google thing found a site that had a checklist of 10 symptoms of silent reflux. Incidentally I was feeding on demand for hours at a time. Doc prescribed gaviscon, 2 days later I realised it was just wind.

She still suffers terribly with wind and despite burping for ages I just couldn't get burps up, she farted (loudly like a bloke) lots, still does and they would frequently hurt her and make her cry out.

She's now 8 weeks, still EBF and things are easier in that I can get burps up now. She obviously gets fractious when feeding, I take her off and wind her till a burp comes up, usually just a couple of mins now.

When she was the age of yours I spent hours online looking for winding methods, some really helped. The bouncing walk for instance.... hold baby to your shoulder and walk in a very exaggerated walk with a spring in your step or going up the stairs. I got a couple of good burps that way.

Good luck getting Infacol in. We used gripe water (from 1 month) but that made her vomit.

If you think a dummy will give you some peace then go for it or help him find his hands to suck on.

Thank you - had a better night last night and I was sure it was just wind but then this morning he is is screaming and arching his back during feeds making me think it is reflux.......

I'd say it sounds like reflux... and also the second poster's child too.
