Experts on growth charts needed, 4.5 month old baby below 2 centile line

Short Story
Worried that DC4 is between the 2nd and 0.4 centile lines on the standard growth charts, following short illness. I thought she was totally healthy and she is growing well, should I be worrying ?

Long Story
Background I have three older children.

DC1 born at 98 centile line dropped and followed 50 centile line
DC2 born at 98 centile line stayed on and followed 98 centile line
DC3 born at 98 centile line dropped to 20 centile line by 6 months but that was the first time she was weighed after the midwives signed us off.

All three children happy, healthy and very different body types.

DC1 average length and size
DC2 butter ball, nearly as wide as he was tall
DC3 thinner than first two and much taller, very long in all clothes.

Then we come to DC4, born on the 98 centile line and I didn't feel the need to get her weighed until a couple of weeks ago, when she got a stomach bug and started throwing up a lot. I was worried as she doesn't have much to lose having the same body type as DC3 long and skinny.

Of course I got her checked out at the doctors who basically said she looked fine and had a stomach bug or maybe reflux but because she was at the light end of healthy to get her weighed twice a week apart at the HV baby clinic, just to check she was still putting on weight despite her sickness.

I went to the clinic the first week and got her weight but I had forgotten my red book, so they wrote it down for me and I took it home and plotted it myself and I was shocked to see she was between the 0.4 and 2nd centile lines !

The following week she was just 3 ozs heavier, however as she had a weeks worth of poo in her on the first weighing and had just done a massive poo just before the second, I reckon she properly but on more than that in reality.

She is no longer throwing up, and had I not got her weighed I would be happy but I am worried for a few reasons.

  1. I was worried that as she is a placid baby, that I had been putting off feeds or cutting feeds short when someone else needed me. Only by a few minutes but that could be enough to have her a feed less over the course of 24 hours, especially as she happily watches her sibling play and she sleeps very well at night as well. I am making efforts to offer much more often and when I have to interrupt a feed, making the effort to top her up asap, rather than waiting for her cues. I think she is far too laid back and I needed to take the control and I am doing so.
  1. I feel bad that deliberately didn't take my red book to the second weighing as I know the HV would of jumped on me as she is less than 5 centile which is the line they watch and I wanted to see if she had put on weight and how she is now she is no longer ill. In my opinion she is healthy, happy and perfectly formed just small like DC3 was, I reckon she'll move up to 20 centile line by 6 months like her sister did. But I doubt the ability for the HV to give me decent advice, especially as in the past I was told the DC1 was perfectly average for being on the 50 centile line and that I should cut back on Exclusively breastfed DC2's milk as he was too big being on the 98 centile. They don't understand that it is not a bell chart distribution chart and it is just as normal to be on any line of the chart, they think the nearer to 'average line' 50 the better...idiots.
  1. I feel guilty as I understand that the reason HV's check out babies in the bottom 5 centile range is in case, those babies are only there due to illness or neglect rather than natural weight and hence should I ask them to look into my baby. But without seeing my red chart and just looking at my baby, the HV's have said nothing but nice positive things about how she looks naked. Plus the doctor and I both feel DC4 is fine and know she isn't throwing up I am no longer concerned.

Should I be worried ?

PS. I will be getting her reweighed next week when the clinic reopens, regardless just to check she is heading along her line.

My DS was 98c at birth and dropped to 3c by around 4 months. He was happy, healthy and meeting milestones. I was referred to Pediatrician and Dietician, who both said nothing wrong.

He started to climb percentiles when he went on to solids.

Is she breast or formula fed? The WHO has different charts for EBF babies.

The HV will be concerned if a baby drops centiles and may want you to take your DD to the GP to investigate what they term as "failure to thrive". This does not mean you are doing anything wrong but they may want to investigate.

Weighing twice a week seems extreme, I think it's normally nonthly at that age, anything more frequent wouldn't give them much to observe.

There is no point in worrying yet, keep getting her weighed regularly and follow their advice. If its any help I am currently going through this with two of my DC, I'm taking it one appointment at a time and doing more than my fair share of worrying but I'm happy that the NHS are able to help me and my DC.

Good luck, I hope your DD regains the weight she has lost and maintains her centiles.

She is breastfed but I really don't want the stress of loads of unnecessary referrals if I can avoid it, plus I'll be weaning her in 6 weeks time anyway. She is super strong and stands up using her own strength when I hold her hands. She will be an earlier walker like her siblings.

The HV can only advise you to take her to the GP. If you are confident there is nothing to worry about at the moment then carry on with the weigh-ins but keep in mind that if she doesn't regain the centiles then you may need to take further action. It is unlikely but not impossible that there could be something else going on for your DD that would be well worth you being aware of.

This is what I don't understand, why does she have to regain a higher weight ?

I am not even sure if she has lost weight whilst being ill, I just thought it was prudent to seek a medical opinion and the doctor thinks she is fine, albeit throwing up at the time. He said he was happy she was a reasonable weight but to get her weighed to ensure she kept her weight and didn't drop any.

I thought it was normal to have catch up OR catch down growth following a healthy pregnancy. I make them big hence they are all 98 centile babies at birth and then they drop to their natural lines and follow them.

Surely if she follows her line around 1 centile that would be good enough for an alert active strong baby who 'talks' to me and plays with objects and meets all her development goals, isn't it ?

I am making sure that, she is getting as much milk as I can fill her with.

I am only returning to the clinic to get her weighed a couple more times on the same scales iyswim.

I won't be taking my red book with me, I'll plot her line myself, when I get home.

As long as she keeps following her line I am happy...if she doesn't, I will make another Doctor's appointment .

She sounds fab, technically she should be referred for a check up due to crossing two centiles. Gp happy though...
You know what you are doing.

It isn't unusual to see what you describe ; a third or fourth baby happy but with lower gain. Maybe due to busy mothers and entertaining siblings? Maybe. Certainly some babies will take much more milk when it is offered then they ask for.

I have 3 DC, youngest is 7 months, they were all born around the 50th. DC1 had dropped to 2nd centile by 20 weeks when I weaned her having seen the GP. DC2 was on her way down by 17 weeks so having seen the same GP I weaned her. DC3 was down too by 17 weeks so I weaned him.

I weaned all 3 before 6 months with medical input, and their weights all then stabilised. DC1 went up a few centiles, as she had been left longer just on EBF than the other 2 who stayed on the lines they were on.

All 3 were alert, happy and slept well.

Thoughts, as you describe there is more than one way to get more calories into a baby. No reason why those calories can't be milk. Supply would rarely be an issue in the scenario described - expectations, routines, illness or just catch down growth much more likely.

If the GP has seen her, there's nothing else a HV would do anyway. Agree that many of them don't understand the charts.
My daughter will be a size 8 when she's grown, like her granny and aunty, (not like me) so why would she be average now??
Her brother looked bony at a higher centile early on because he is much stockier. He did have a problem getting enough milk, and was clearly healthier when he'd got up past 75% centile. DD would be obese at that weight as she's about half as wide.
Point being - the charts are a very blunt tool and mean nothing by themselves.

DD2 was born a bit above the 9th percentile fell totally off the graph between 4 and 5.5 months.

Paediatrition could find nothing wrong.

She totally refused to take a bottle, so was weaned slightly early with lots of yoghurt.

By one year she was on the 50% line, She's almost 12 and still exactly average size. She still wont drink milk
